Bernhardt Dressers and Nightstands Recall: Bernhardt Furniture Company is recalling its Marquesa mirrored glass front dressers with model number 359-044 and Marquesa mirrored glass front nightstands with model number 359-234. The recalled dressers are unstable if they are not properly anchored to the wall, posing a serious tip-over and entrapment hazard that can result in death or injuries to children. Both models have a gray cashmere finish and mirrored glass drawer fronts with metal overlays. The series and style numbers are printed on a white label on the back of the product. The first three characters of the number will be 359 followed by the style number. The products included are found here. These products were sold at Independent furniture stores nationwide from March 2015 through May 2016. Consumers should immediately stop using any recalled dressers and nightstands that are not properly anchored to the wall and place them into an area that children cannot access. Kabob’s Poultry Products Recall: Kabob’s Acquisition, Inc. is recalling approximately 44,850 pounds of raw intact and heat treated, not ready-to-eat (NRTE) meat and poultry products that may be adulterated with E. coli O121. The not ready-to-eat meat and poultry hors d’oeuvres items were produced on various dates between Dec. 8, 2015, and Jan. 15, 2016. The full list of recalled products can be found here. The products subject to recall bear establishment number “Est. 6640” or “P-6640” inside the USDA mark of inspection. These items were shipped to hotel, restaurant, and institutional locations nationwide. Consumers who have purchased these products are urged not to consume them. General Mills Vegetarian Indian Curry Recall: General Mills is voluntarily recalling a limited quantity of frozen Beyond Meat Vegetarian Indian Curry with Beyond Chicken, produced over seven days in April 2016. This product is available exclusively at Whole Foods. The recall is being issued as a precaution after the producer of frozen peas used as an ingredient in this product issued a national recall for the potential presence of Listeria monocytogenes. This voluntary recall is limited to only one variety of frozen Beyond Meat product — Vegetarian Indian Curry with Beyond Chicken — and only with the following “Better if Used By” dates printed on the package: 10NOV2016 NL, 12NOV2016 NL, 15NOV2016 NL, 16NOV2016 NL, 20NOV2016 NL, 24NOV2016 NL, and 25NOV2016 NL. Consumers are urged to dispose of the products affected by this recall. Recall Date: 07/11/2016
Alere Monitor Systems Recall: Alere Inc. is initiating a voluntary withdrawal of the Alere INRatio® and INRatio®2 PT/INR Monitoring System. In certain cases, the system delivers a result that differs from that of another measurement method. Alere is working with the FDA to determine the most appropriate timing for product discontinuation and will provide guidance on transitioning patients to an alternate solution to allow them to continue anti-coagulation monitoring in the least disruptive manner possible. Although Alere is confident that the software enhancements it developed and submitted to the FDA at the end of 2015 effectively address this issue, the FDA notified the company that it believes the company’s studies do not adequately demonstrate the effectiveness of the software modification and advised Alere to submit a proposed plan to voluntarily remove the INRatio® device from the market. The company suggest that patients speak with their healthcare providers prior to making any changes to their current PT/INR monitoring practices. Recall Date: 07/12/2016
Betty Crocker Cake Mix Recall: Two flavors of cake mix — Betty Crocker Delights Super Moist Party Rainbow Chip Cake Mix (called Betty Crocker Super Moist Rainbow Bit Cake Mix in Canada) and Betty Crocker Delights Super Moist Carrot Cake Mix — have been added to the recall as they contain flavor chips (or bits) produced using Wondra flour, which was included in General Mills’ previous July 1st recall announcement. General Mills continues to collaborate with health officials to investigate a multistate outbreak of E. coli O121 connected to several brands of flour. Guidance from the FDA and the Centers for Disease Control (CDC) continues to be that consumers should refrain from consuming any raw dough or batter products. Simmons Prepared Foods Chicken Products Recall: Simmons Prepared Foods, Inc. is recalling approximately 5,850 pounds of frozen, heat treated, not ready-to-eat (NRTE) chicken products that may be contaminated with E. coli O121, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced. The problem was discovered on July 7, 2016, when Simmons Prepared Foods, Inc. received notice from a supplier that flour sold to the establishment was recalled by General Mills. The firm used the recalled flour to bread the chicken fritters affected by this recall action. The frozen, heat treated, not ready-to-eat (NRTE) chicken tenderloin items were produced on Jan. 25, 2016. The products included in the recall include: 30-lb. net-weight case containing six, 5-lb. bags in clear film of “Simmons UNCOOKED CHICKEN TENDERLOIN FRITTERS,” with a case code 31473, packaging date code of 6025, and a Use-By date of 01/25/17 and 30-lb. net-weight case containing six, 5-lb. bags in clear film of “Simmons UNCOOKED CHICKEN BREAST TENDERLOIN FRITTERS,” with a case code 62331 and a packaging date of 6025. The products subject to recall bear establishment number “P-5837” inside the USDA mark of inspection. These items were distributed to Arkansas for institutional use. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase. Recall Date: 07/12/2016
Flushmate III Pressure-Assisted Flushing Systems Recall: Flushmate, a division of Sloan Valve Company, is recalling its Series 503 Flushmate III Pressure Assist flushing systems installed inside toilet tanks that were manufactured from July 2009 through April 2011. The system can burst at or near the vessel weld seam releasing stored pressure. This pressure can lift the tank lid and shatter the tank, posing impact and laceration hazards to consumers and property damage. Previously recalled systems were manufactured from October 1997 through June 2009. Recalled units have a date code/serial number that is 16 characters long and is located on the label on the top of the Flushmate III. The first six numerals of the serial number are the date code. The date code range for units included in this recall in MMDDYY format is 070109 (July 1, 2009) through 043011 (April 30, 2011). The date code range for previously recalled systems in MMDDYY format was 101497 (October 14, 1997) through 063009 (June 30, 2009). Units included in this recall were sold individually and installed in toilets manufactured by American Standard, Copperfit, Crane, Ecotech, Eljer, Gerber, Kohler, Lamosa, Mancesa, Mansfield, Orion, St. Thomas, Universal Rundle, Vitra, Vitromex, Vortens and Western Pottery. These products were sold at Home Depot and Lowe’s stores, toilet manufacturers, distributors and plumbing contractors nationwide from 2009 through 2011. Consumers should immediately stop using the recalled Flushmate III systems, turn off the water supply to the units, flush the toilet to release the internal pressure and contact the firm to request a free repair kit. Home Source Floor Lamp Recall: Home Source Industries LLC is recalling its brown metal floor lamps with alabaster glass shades. The lamps can short circuit due to defective sockets, posing fire and shock hazards. They are illuminated with a single 100-watt light bulb and measure about 70 to 72 inches tall. The model numbers LMP4229, LMP4168 and LMP10771 can be found on the label at the bottom of the lamps. These products were sold at small retail stores located in the New York metropolitan area and online at Amazon.com, Houzz.com, Target.com and Wayfair.com from February 2016 to April 2016. Consumers should immediately stop using the recalled floor lamps and contact Home Source for information on returning the lamps for a full refund. Recall Date: 07/14/2016
Liquid Plumr Clog Removers Recall: The Clorox Company is recalling three types of Liquid Plumr Clog Removers. The recalled bottles are not child-resistant and children can remove the top, posing a risk of chemical burns and irritation to the skin and eyes. These products contain sodium hydroxide which is required by the Poison Prevention Packaging Act to be sealed with child-resistant packaging. This recall involves three Liquid Plumr clog removing products, sold in 17 ounce bottles that are about 9 inches tall. The bottles have the Liquid Plumr logo and the name of the product on the front. Liquid Plumr products included in the recall are: Liquid Plumr Pro-Strength Foaming Clog Fighter, formerly sold as Slow Flow Fighter (UPC Code 44600-00214; orange bottle with red cap), Liquid Plumr Pro-Strength Urgent Clear (UPC Code 44600-30548; blue bottle with a red cap), and Liquid Plumr Industrial Strength Urgent Clear (UPC Code 44600-31484; black bottle with a red cap). Only bottles manufactured before March 22, 2016 are included in this recall. These products were sold at Dollar General, Family Dollar, HEB, Home Depot, Kroger, Lowe’s, Meijer, Publix, Rite-Aid, Target, Walgreens, Walmart and other dollar, grocery, hardware and mass merchandise stores nationwide and online at Amazon.com, Drugstore.com and Quidsi.com from January 2012 through May 2016. Consumers should keep this recalled product away from children and immediately contact The Clorox Company’s Liquid Plumr call center for disposal and refund instructions. Recall Date: 07/14/2016
IKEA Safety Gate and Safety Gate Extensions Recall: IKEA North America Services LLC is recalling IKEA PATRULL, PATRULL FAST and PATRULL KLÄMMA safety gates and safety gate extensions. The locking mechanism can open unexpectedly, posing a fall hazard to children and other consumers. The pressure-mounted gates open both inward and outward. They measure about 28 ¾ inches high with an adjustable width from about 28 ¾ inches to 34 ¼ inches. The gate extensions measure about 28 ¾ inches high with an adjustable width from about 2 ½ inches to 5 inches. A permanent label is attached to the metal bar at the bottom of the safety gate containing “IKEA” and the article number. Gate article numbers included in this recall can be found here. These products were sold at IKEA stores nationwide and online at www.ikea-usa.com from August 1995 through June 2016. Snack Factory Original Pretzel Crisps Recall: Baptista’s Bakery, Inc. announced a voluntary recall of a limited number of 30 oz. Snack Factory® Original Pretzel Crisps® packages because they may contain undeclared milk ingredients. People who have an allergy or severe sensitivity to milk run the risk of a serious or life threatening allergic reaction if they consume the affected product. This voluntary recall covers only 30 oz packages of Snack Factory® Original Pretzel Crisps® (UPC code 049508250401; Best By Date: 07-01-17). Baptista’s Bakery, Inc. is initiating this recall out of an abundance of caution after determining that seasoned product produced in the same facility may have been commingled with the product listed above. IKEA Chocolate Products Recall: IKEA North America Services LLC is recalling seven types of chocolate products due to traces of undeclared milk, hazelnuts, and/or almond. IKEA had samples of chocolate analyzed and found them to contain milk, hazelnut and/or almonds. Persons allergic to milk, hazelnuts and/or almonds may have a severe to life-threatening reaction after consuming the dark chocolate. Products have been distributed Nationwide at IKEA retail stores and sold individually from April 30, 2015 to July 1, 2016. The full list of affected chocolate products can be found here. Golden Dipt Jalepeño Breaders Recall: Kerry Inc. is recalling two lots of Golden Dipt® Jalapeño Breader that were sold to foodservice distributors because one of the suppliers recalled an ingredient used in our mix for the potential presence of E. coli O121. Kerry Inc. was notified by the supplier that two of their products, a red bell pepper nugget and a jalapeno nugget, were made with a small percentage of affected flour which was recalled by General Mills because it may be contaminated with E. coli O121. The affected Golden Dipt® Jalapeño Breader is in a white 10 pound bag with the UPC number 763089721548 and Item Number: G5620.690. The recall only affects lots 0414647003 and 0513647003. These two lot codes of Golden Dipt® Jalapeño Breader were manufactured by Kerry Inc. and sold to foodservice distributors located in Florida, Texas, Oklahoma, Illinois, Minnesota, New Jersey, Missouri and regions of Canada. Product may have been further distributed beyond these locations throughout the foodservice industry and to retail stores open to the general public. If you have purchased Golden Dipt® Jalapeno Breader with either of the lot codes noted above, please dispose of the product and seek reimbursement with the distribution group/store where it was purchased. Michael Angelo’s Shrimp Scampi Recall: Michael Angelo’s Gourmet Foods, Inc. is recalling approximately 4,225 pounds of frozen shrimp scampi products due to misbranding and undeclared allergens. The products may be mislabeled and contain chicken, as well as egg and soy, known allergens, which are not declared on the product label. The product subject to recall includes 26-oz. boxes, containing plastic trays, labeled, “Michael Angelo’s Uncompromisingly Italian-Style Shrimp Scampi with Linguini Pasta in a Fresh Heirloom Garlic-Butter Sauce” with a packaging date of May 7, 2016, “BEST IF USED BY” date of May 7, 2017 and UPC code 0 37363 98377 0. Due to a packaging error, products are labeled as, “Michael Angelo’s Uncompromisingly Italian-Style Shrimp Scampi with Linguini Pasta in a Fresh Heirloom Garlic-Butter Sauce;” however, the packages may contain a chicken piccata product, which contains chicken, egg and soy, ingredients not declared on the label. The products subject to recall do not bear the USDA mark of inspection, as shrimp products are regulated by the U.S. Food and Drug Administration (FDA). These items were shipped to distributors in the Southeastern U.S. Consumers who have purchased these products are urged not to consume them. Marie Callender’s Cheese Biscuit Mix Recall: International Commissary Corporation (ICC) has issued a voluntary recall of certain lots of two retail products: Marie Callender’s 7oz & 14oz Cheese Biscuit Mix. The 7 oz recalled product has Best By dates of 3/22/17 and 5/17/17 and was sold in retail stores in the following states – AL, CA, WA, UT, and TX. The 14 oz recalled product has a Best Buy date of 6/17/17 and was sold only in Stockton, CA. ICC was notified by a supplier that an ingredient contains flour which was recalled by General Mills because it may be contaminated with E. coli O121. Consumers who have purchased any of the recalled products are urged to throw them away. Retailers who received the recalled products are being contacted. FDA and the Centers for Disease Control (CDC) warn that consumers should refrain from consuming any raw products made with flour. This recall is being made with the knowledge of FDA. Nestlé AccuPure Water Dispenser Recall: MTN Products, Inc. is recalling its AccuPure countertop filtered water dispensers that were distributed by Nestlé Waters North America, Inc. due to reports of parts melting, smoking or burning. The water dispensers are black and measure about 17.5 inches tall and 12.8 inches wide. The units connect to the plumbing system, draw water from the system, filter the water and dispense hot and cold water through the unit. “AccuPure” is printed on the front and side panels. The model number is HCB215n and is printed on the back of the water dispensers. These units were sold and rented to businesses nationwide for consumer use from January 2013 to May 2016. Consumers should immediately unplug the water dispensers and stop using them. Nestlé Waters is contacting affected customers directly to schedule installation of a free replacement unit. Poppers Mozzarella Cheese Sticks Recall: Monogram Appetizers is recalling approximately 5,000 cases of Poppers Brand Mozzarella Cheese Sticks because the product may contain undeclared egg. The recall was initiated after it was discovered that the product was distributed in packaging that did not reveal the presence of egg. People who have an allergy or severe sensitivity to egg run the risk of serious or life-threatening allergic reaction if they consume these products. The recalled Poppers Brand Mozzarella Cheese Sticks were distributed nationwide in retail stores. The product comes in a 32 ounce, clear plastic package marked with UPC 4670488920 and code numbers. Consumers who have purchased 32 ounce packages of Poppers Mozzarella Cheese Sticks with the referenced code numbers are urged to return them to the place of purchase for a full refund. Siluriformes Fish Products Recall: U.S. Cado Holdings, Inc. is recalling approximately 1,650 pounds of imported, frozen Swai fillets products that were distributed into U.S. commerce without meeting federal importation requirements, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced. The Deep Water Sea Food brand Frozen Swai fillets were produced on April 5, 2016 and were imported from Vietnam and failed to comply with FSIS requirements concerning residue sampling and testing prior to entry into United States commerce. This release is being reissued as an expansion of the June 20, 2016 and June 25, 2016 recalls to include an additional 1,650 pounds of frozen Swai Fillets. The combined number of recalled product from June 20, 2016 to July 14, 2016 is 29,645 pounds of frozen Swai imported from Vietnam. The full list of affected fish can be found here.
