Position Description Empath Health is a nonprofit integrated network of care that provides expert, life-changing health care to those with chronic and advanced illnesses. Empath Health includes hospice, palliative home health and physician services, independent living assistance and day care for seniors, HIV services and advance care planning. Empath Home Health is a nonprofit provider of comprehensive home health care services based in Pinellas County. We are currently seeking an experienced and compassionate Liaison to act as a consultant to referral sources educate them about Palliative Home Health Services. The Liaison is knowledgeable in State and Federal Home Health guidelines, as well as adept at educating the professional community. He/she will identify and facilitate opportunities for an increased palliative home health presence and collaborations with healthcare providers which will improve access to Empath Home Health and related programs. Position Requirements As a Professional Relations Liaison with Empath Health you must be patient/family focused, loyal, motivated and interested in serving your community. Other requirements of this role include: * Education and/or Experience: RN (Registered Nurse) or LPN (License Practical Nurse) with current license in the State of Florida. * Licensure: Active and current State of Florida Registered Nurse license or Practical Nurse license. * Professional relations and/or sales experience. * Minimum 5 years clinical expertise in related discipline. * Excellent communications and interpersonal relationship skills, including public speaking experience. * Must have reliable transportation/insurance and be willing to travel throughout the county. * Ability to work unsupervised, exercising a high degree of discretion and independent judgement. * Basic computer skills with a willingness to learn; familiarity with spread sheet programs.
