Carla Chmielewski of Holland, MI, passed away at her home on Monday, March 13, 2017.
Born on April 30, 1952 to Ivan and Nora Cook, who predeceased her, Carla was a graduate of Holland High school.
Carla is survived by her dedicated and loving husband, Fred, and their Golden Retriever, Annie; Daughters Leigh (Jonathan) Levine and Shelley Arendshorst; Granddaughters Katherine Levine, Rebekah Levine, Brynne Buchanan and Ingrid Buchanan; Brothers Roger Cook and Doug Cook.
