This service is provided for persons needing to replace their lost, stolen or damaged driver's license.

Applicant must be the holder of a valid driver's license.
==Instructions==
Please provide instructions on obtaining the certificate/documents.
e.g. The state office holds birth records since january 1908.

*Full Name
*Date of Birth
*Address
*Previous Address
*Last Renewal Year
==Procedure==
#Applicant must obtain a police report from their neighborhood police station.
#Applicant must then complete a research information form.
#Applicant should then present completed information form and a police report to the driver’s license unit at the road Traffic Department.
#The Driver’s License Clerk will research the Driver’s License.
#Following the research process, the research Clerk will submit the applicant’s records to the driver’s license clerk for duplication (if valid) or for renewal.
