Individual will be responsible for providing recommendations and/or assuming responsibility for nutritional support, pharmacokinetics, or other medication regimens as prescribed by physician\. Responsibilities will include, but not be limited to: Participate as a member of the Pharmacy and Therapeutics \(P&T\) committee Identify target medications/therapies, develop criteria, and implement data collection for Medication Use Evaluations \(MUE\)\. Tabulate and present completed MUE data to the P&T committee\. Assist in formulary maintenance by critiquing new entities, comparing medication classes, and developing therapeutic interchanges where applicable\. Develop and provide educational programs/inservices for health care practitioners and patients as required\. Assist in preparing for and maintaining compliance with regulatory agency \(e\.g\. JCAHO\) standards/requirements\. ¢ Participate on assigned committees\. ¢ Meets and talks with pharmaceutical sales representatives as time permits ¢ Document clinical interventions on an ongoing basis\. ¢ Demonstrate a basic understanding of distributive responsibilities including computer functions\. ¢ Assist in investigational drug studies when needed\. ¢ Provide drug information when needed or requested\. ¢ Upon physician request, provides recommendations and/or takes over therapy providing coverage of nutritional support, pharmacokinetics, or other medication regimens as prescribed by the physician\. ¢ Provide on\-call clinical pharmacy services as needed\. ¢ Assist in developing drug protocols\. c Participate as a preceptor/instructor for students from accredited colleges/schools of pharmacy Required Minimum Education: ¢ Graduated from an accredited college of pharmacy ¢ Licensed by, and in good standing with, the Georgia State Board of Pharmacy Required Minimum Experience: ¢ Two years clinical experience and/or completion of a hospital residency ¢ Experience with nutritional support and pharmacokinetics Required Skills: ¢ Strong verbal and written communication skills ¢ Strong computer skills\. Experience with STAR, Microsoft Office, etc… preferred\. ¢ Capable of preparing and providing presentations ¢ Strong organizational skills

Required Minimum Education:

99. B\.S\. from an accredited School of Pharmacy, Pharm\.D\. preferred
99. Licensed by, and in good standing with, the Georgia State Board of Pharmacy

Required Minimum Experience:

ØCompletion of a PGY\-1 Residency plus at least one year clinical experience\.

ØExperience with nutritional support, anticoagulation management and pharmacokinetics preferred

Required Skills:

ØStrong verbal and written communication skills

ØStrong computer skills\.Experience with Epic, Microsoft Office, TheraDoc etc\. preferred\.

ØCapable of preparing and providing presentations

Strong organizational skills
