The Liaison Nurse provides education and coordination between Wellstar Community Hospice and various System and community referral sites /resources including physicians, hospital and health facilities and community based health programs and hospice whether inpatient or home based\. Identifies and maintains a list of professional resources; develops and implements a plan to provide education concerning the services available through hospice\. Identifies unmet needs that can be addressed through hospice\. Identifies process improvement opportunities based on physician and community feedback; works with the Director and clinical leadership to develop and implement needed changes\. Reports administratively to the Executive Director of WellStar Community Hospice; functionally to the Hospice Assistant Manager of Home Hospice\. Maintains a professional appearance\.
**Required Minimum Education** : Graduate of an accredited school of nursing\. BSN or other
Baccalaureate degree desired\.
**Licensure** : Current Georgia license as a registered nurse\.
**Required Minimum Experience** : Documented evidence of above average clinical hospice knowledge or Medicare criteria, Documented experience in identifying customer needs and modifying processes to accommodate those needs, Hospice experience required
**Required Skills** : Possesses emotional maturity Strong leadership, problem solving, team building and timemanagement skills\. Demonstrates ability to work as a member of an interdisciplinary team\.Able to work independently and to manage multiple priorities\.Positive, very strong customer oriented interpersonal skills\-patients/caregivers, referral sources\- care coordinators, Post Acute Navigators, floor nurses\. Ability to coordinate, communicate, and make all necessary arrangements for the patients transportation, and care at home or in their choice of residence\. This may require coordination with other facilties, i\.e\. the patient /family choice of Nursing Home, ALF, etc\. Or admit to Hospital GIP level of care – hospital documentation as well as hospice\. Knowledge of multiple facets of healthcare system relating to the provision of hospice care\. Strong written and verbal communication skills including strong group presentation skills\. Experience using automated systems and common personal
computer applications \(word processing, spreadsheet software\)\.Cerner program skills\- admission\.
