EVOLVE – PART OF CREST WALTHAM FOREST is looking for a primary care mental health navigator to work with people with a long term mental health condition following discharge from secondary care mental health services.

SALARY:  £26,000 per annum – fixed term contract until 31.3.15

This unique and ground breaking role requires candidates to have experience of working with adults who have a long term mental health condition and a working knowledge of the challenges and support needs of this client group.  A thorough understanding of Recovery with the skills and ability to motivate and empower people are also required.

Then using a person-centred and outcome focussed approach alongside excellent communication skills, you will:

support clients to access their primary care mental and physical health checks regularly and navigate healthcare pathways efficiently in a range of primary care settings throughout the borough of Waltham Forest

support clients to access community services/organisations to ensure social inclusion as part of their daily lives including, education, volunteering, employment, healthy living, faith and spirituality and the arts

work alongside primary care teams to improve access to physical and mental health services which will be measured by improved health outcomes

adopt a person-centred approach to promote empowerment and self advocacy on issues that include health, housing, benefits and financial management including debt

You will need to:

demonstrate excellent communication skills to enable you to interact with clients from diverse backgrounds and liaise with a wide range of professional services/organisations

have a good self-awareness when working with people who demonstrate levels of emotional distress

be able to work under pressure and meet outcome targets

ideally be educated to degree level but with minimum qualifications at A level (or equivalent)
