A managed care company is filling a position for a Telecommute Case Manager in Irving. Core Responsibilities of this position include: Providing case management services to members with chronic or complex conditions Promoting integration of services for members including behavioral health and long term care to enhance the continuity of care for members Maintaining department productivity and quality measures Skills and Requirements Include: Bachelor’s degree in nursing or Master’s degree in social work, counseling, health education or other mental/social health discipline or a combination of experience and education 3 to 5 years of clinical experience 3+ years of case management experience Active and unrestricted State Registered Nursing license or Licensed Clinical Social Worker, Advanced Practice Social Worker, Licensed Professional Counselor or Licensed Marriage and Family Therapist in good standing Knowledge of applicable state and federal regulations
