All the Stenographers (BPS-16) may be requested to got forward their Record of Service i.e. Service Book and Leave Account duly completed from their departments to their respective Audit Offices for maintenance of their Record of Service i.e. History of Service and Leave Account and issuance of their Pay Slips under the following rules and regulations: -

RECORD OF SERVICE

S.R. 196.

A record of the services of a gazetted Government servant will be kept by such audit officer and in such form as the Auditor General may prescribe.

S.R.    215(a).

The leave account of a gazetted Government servant shall be maintained by, or under the direction of, the principal auditor responsible for the audit of his pay.

RECORD OF SERVICE

B-16 and above Government Servants (S.R. 196):

The record of the service of a BPS-16 & above officers is kept by the respective audit officer in such form as may be prescribed by the Auditor General of Pakistan.  In pursuance of these powers the Auditor-General has decided that a ‘History of Service’ will be maintained for every gazetted officer, which will contain the following information:-

Name; Qualification; Date of birth; Date of joining service; Domicile; Province of origin; Mother tongue; Stations of appointment; Dates of officiating substantive appointments; Leave and suspensions, etc.

Any literary work done by the Government servant or any title conferred upon him are also mentioned therein.

Respected Sir,

It is
submitted that on upgradation of the post of Stenographer from BS-15 to BS-16
with effect from 23.12.2011, vide Finance Division (Regulation Wing)’s O.M. No.
19(55)Legal-II/2010-1055 dated 23.12.2011 (copy enclosed), the posts of
Stenographers in the ______________________ were upgraded alongwith incumbents
accordingly, vide _________’s notification No. __________ dated ___________
(copy enclosed), I assumed the charge of the upgraded post, vide __________’s
Charge Assumption Notification No. ______________ dated __________ (copy
enclosed alongwith charge assumption report).

2.         It
is further submitted that under SR 196 & SR 215(a), the Record of
Service i.e. History of Service and Leave Account of gazetted Government
servants are required to be maintained at audit office, therefore, the
following relevant required documents are attached (in triplicate) : -

Specimen
signatures duly attested.

(ii)

Finance
Division (Regulation Wing)’s O.M. No. 19(55)Legal-II/2010-1055 dated 23.12.2011.

(iii)

view of the above, it is requested that the Record of Service i.e. Service

may kindly be sent to AGPR (GA-IV Section), Islamabad through DDO, ________ for
maintaining Record of Service i.e. History of Service & Leave Account, etc.
at their level and issuance of Pay Slip.
