The listings give the health inspector score, if applicable, along with the text of their findings in their words.

Riverwood Assisted Living Facility food services, 711 W. Atkins St., Dobson. Inspected Dec 14, score 97.5. Violations: 1) Food-contact surfaces: cleaned and sanitized: Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils – P — A few dirty ladles and bowls were found placed up considered ready-to-use. After dish washing employees must check to ensure that all food has been removed from the dishes, before placing them up. These were washed to correct this.

2) Non-food-contact surfaces clean: Nonfood Contact Surfaces – C — Clean inside of the short 2-door cooler. Clean shelving where needed. Clean the top of the tea brewer in the dining room.

3) Physical facilities installed, maintained and clean: Floors, Walls and Ceilings-Cleanability – C — Repair the ceiling where damaged (above the 2-door reach-in cooler and beside of the hood system). Reattach baseboard to the wall where falling (dining room). Cleaning, Frequency and Restrictions – C — Clean the floors underneath and behind equipment where needed. Clean the waste pipes where needed, especially at the dish machine.

Riverwood Assisted Living Facility rest/nursing home, 711 W. Atkins St., Dobson. Inspected Dec 14, score 92.5. Violations: 1) Floors easy to clean, no obstacles, drains where needed: Clean the window sill in the barber shop room. Remove the caulk and reapply in the shower room behind the med room (mold). Sweep in the corners of the living room. Repair the chipping wall in the laundry room. Sweep the floor in the hall closets and supply closet. Clean the floor under beds where needed. Reattach baseboard where fallen (employee restroom, shower room behind med room, room 21). Clean cobwebs from closets where needed. Repair broken floor tiles where needed (bathroom between rooms 19 and 21, shower room between rooms 11 and 13, room 28).

2) Ambient air temperature 65 to 85 F, equipment clean: Ventilation equipment shall be kept clean and in good repair.—Clean the vents in the ceilings where needed (shower room, employee restroom). Keep light fixtures clean (resident rooms, restrooms).

3) Facilities conveniently located, clean and in good repair: “All institutions shall be provided with toilet, handwashing, and bathing facilities that are conveniently located and readily accessible to residents and staff. These facilities, and laundry facilities when provided, shall be kept clean and in good repair.”—Clean the toilets and sinks where needed (hallway shower room). Unclog the handwash sink in the restroom between room 33 and 35. Clean the tub in the shower room behind the med room (now used for storage). Clean the top of the dryer. Clean the shelving in the laundry room.

4) Lavatories have mixing faucet or tempered water, soap hand towel and hand drying device: All lavatories shall be supplied with hot and cold running water through a mixing faucet, or with tempered warm water, soap, and sanitary towels or hand-drying devices.—Two of the paper towel dispensers had a low battery and was not dispensing paper towels (restrooms between rooms 20 and 22 and between rooms 33 and 35). Replace the batteries.

5) Lavatory and bathing hot water between 100 and 116 F: Water supplied to 2/3 of the establishment is reaching and maintaining only 93-95 degrees F. Hot water must be maintained at 100-116 degrees F.

6) Vermine excluded: “Effective measures shall be taken to keep flies, rodents, cockroaches, and other vermin out of the establishment and to prevent their breeding or presence on the premises. All openings to the outer air shall be protected against the entrance of flies and other flying insects by self-closing doors, closed windows, 16-mesh or finer screening, controlled air currents, or other effective means.”—Flies are numerous, especially in the laundry room. Look into fitting the door with a self-closing arm to prevent flies from entering the establishment. Also, a stray cat wandered into the laundry room. Keep animals out of the facility (unless they have the proper vet records).

7) Patient contact items in good repair, properly stored, cleaned and disinfected: “Shared bathing equipment that has contact with patient’s skin shall be cleaned with detergent and an EPA registered hospital disinfectant between patient uses.” —Disinfectant is not being applied to bathing equipment and bathing facilities after each patient. An EPA-registered disinfectant must be applied, following manufacturers’ instructions, between patient uses. Disinfectant is available, however. The shower chair in the shower room behind the med room is dirty and needs to be cleaned and disinfected. Clean and disinfect the shower curtain in the same room. “All furniture, bed springs, mattresses, sleeping mats, draperies, curtains, shades, venetian blinds, or other furnishings in institutions shall be kept clean and in good repair.”—Replace the couch in the living room (damaged).

Sue S, 911 East Atkins St., Dobson. Inspected Dec. 15, score 92.5. Violations: 1) Hands clean and properly washed: When to Wash – P- Observed several employees changing gloves without washing hands. When changing tasks/gloves hands must be washed before putting on new gloves. Cdi by pic discussing with the employees.

2) Food-contact surfaces: cleaned and sanitized: Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils – P- Clean and sanitize the ice machine following manufacturer’s instructions. Tubing needs to be changed out. Moldy. I will return within 10 days for compliance. Clean and sanitize the can opener. Cdi by sending to be washed.

3) Gloves used properly: Gloves, Use Limitations – C- Observed an employee crack an egg and not change gloves. Gloves must be changed when working with raw food and ready-to-eat food to avoid contamination. Cdi by removing gloves and washing hands and putting on new gloves.

4) Equipment, food and non-food-contact surfaces approved; cleanable, properly designed, constructed and used: Good Repair and Proper Adjustment-Equipment – C- Repair the tall one-door reach-in cooler door handle on the cook line. Do not use tape. Not easy to clean. Gasket in bad repair on the sandwich unit. Replace the gasket on the walk-in cooler door and the one-door reach-in. Sand and paint rusty prep table shelving. Shelving in the walk-in cooler and salad cooler are rusty and needs to be repaired or replaced.

5) Warewashing facilities: installed, maintained and used; test strips: Manual Warewashing Equipment, Wash Solution Temperature – PF- Observed an employee washing dishes in water that was 99 f. When actively washing dishes the detergent water must be at least 110 f. Cdi by changing the water.

6) Non-food-contact surfaces clean: Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils-clean sides, legs, tops, and feet of equipment for dust, grease, and crumbs. Clean inside refrigeration and freezers throughout. Special attention in the grill and fryer areas. Heavy grease build-up. Clean shelving in reach-in units and walk-in cooler. Clean the outside of the dry good containers. Clean the fan. Clean fryers.

7) Physical facilities installed, maintained and clean: Repairing-Premises, Structures, Attachments, and Fixtures-Methods – C- Repair baseboard where it is coming off the wall. Re-grout floors where needed. Replace missing baseboard tiles (cooking area). Tighten the toilet seats in the woman’s restroom. Bathroom stall rusty beside the urinal in the men’s restroom. Cleaning, frequency and restrictions – c- clean floors, walls and ceilings throughout. Heavy cleaning needed under/behind equipment. Clean the mold behind the prep sink/table. Cleaning needed in floor sinks. Clean walk-in floors. Clean bugs from light fixtures. Clean ceilings throughout. Clean can wash area walls.

8) Meets ventilation and lighting requirements; designated areas used: Intensity-Lighting – C- Replace burned out lights in the kitchen. Lighting needs to be at least 10 foot candles in the walk-in cooler and freezer. Cleaning Ventilation Systems, Nuisance and Discharge Prohibition – C- Clean the hood. Clean air vents.
