Ventenne di Edimburgo, Mark Renton ha scelto per sé "un'onesta e sincera tossicodipendenza" in modo da non doversi preoccupare del sesso, del lavoro o dei rapporti sociali. Ha degli amici o meglio dei compari con cui rubacchiare, parlare a vuoto, bere birra e bucarsi le vene: sono Sick Boy, tutto provocazioni e capelli ossigenati, Begbie, imprevedibilmente violento e alcolizzato, Tommy, che cerca di star fuori dal giro dell'eroina, e Spud, forse l'unico di cui può fidarsi. Tra tentativi di disintossicazione, ricadute e un inatteso colpo che può cambiare la vita a tutti, Mark forse entrerà a far parte dell'odiata, ma rispettabile società.

Nel 1999 il British Film Institute l'ha inserito al decimo posto della lista dei migliori cento film britannici del XX secolo. Nel 2004 il film è stato definito come il miglior film scozzese di tutti i tempi in un sondaggio di pubblico generale.

Nel 2017 è prevista l'uscita del sequel diretto nuovamente da Danny Boyle con lo stesso cast del film originale.
