The primary responsibility of the RN Case Manager is to identify, screen, track, monitor and coordinate the care of patients with multiple co-morbidities and/or psychosocial needs and develop a case management plan of care for high-risk patients. This position will be utilized to focus on the full spectrum of needs of high-risk individuals to reduce risks and improve their health, including enabling and coordinating access to covered services; acquisition of needed medical supplies and equipment; health education and coaching; and home health services as needed. This position is based in the Yakima, Washington area, but will travel as needed throughout the Yakima Valley and Spokane areas.

Additional duties include but are not limited to:

• Assist patients in accessing community resources to help them manage and improve their complex health issues, providing equal focus of intervention and coaching.

• Identify appropriate alternatives to hospitalization, and help patients navigate the health care system, educating them about options.

• Promote continuity of care.

• Establish communication with other team members, providing information to care teams and patients as required, serving as an intermediary between patients and providers.

• Maintain high levels of patient and practitioner satisfaction of coordinated care.

• Assure that services are delivered at the appropriate level of care in a timely, effective, and cost efficient manner.

• This position requires 5 or more years of RN case management experience, a current RN license, and Case Management Certification (CCM).

QUALIFICATIONS:

Education: Bachelor’s Degree in Nursing.

Experience: Five or more years of directly relevant case management experience. Diverse clinical experience in caring for acutely ill patients with multiple disease conditions is preferred. At least 9 years of directly related experience can be substituted for the bachelor degree.

Licenses/Certificates/Registration: Case Management Certification (CCM) is required. Registered Nurse with current licensure to practice in Washington State or ability to obtain Washington State RN license prior to starting work; Current driver’s license and proof of automobile liability insurance coverage. Bilingual in English/Spanish preferred but not required. Must demonstrate ability to communicate at level 10 on the ALTA language proficiency test to be eligible for bilingual differential.

Knowledge/Skills/Abilities Required: Knowledge of utilization management, quality improvement, discharge planning, and cost management. Ability to work independently; ability to work with culturally diverse populations; ability to communicate effectively orally and written; knowledge of human service programs and the ability to develop effective working relationships with clients, social service providers and partners; ability to work some evening and weekends.
