Position:  Family Team Conference Facilitator – Sebring

We are currently seeking a full-time Family Team Conference Facilitator in Sebring, FL.  This position is responsible for implementing permanency planning practices as it relates to searching and assisting with family connections for the children in the system of care. This position will identify and engage family members and/or life-long supportive adult connections.

Requirements:
◾Facilitate Family Team Conferences to ensure the children served by OHU are able to achieve permanency timely and efficiently.
◾Responsible for providing direct assistance, consultation and guidance to case management staff within our system of care related to search and discovery of family members or non-relative supportive adults.
◾Coordinate the collection and development of performance and outcome data reports, aggregation and trends for performance management and outcome management, including contract performance measures; analyze data and report findings.
◾Successfully transition youth to a least restrictive family like setting with pathways to permanency and enhanced well-being.
◾Assist the Case Management Teams in preparation and implementation of Family Team Conferences, and/or Family Team Meetings.
◾Provide linkages to foster parents, adoptive parents, therapeutic foster parents or other alternative living arrangements to support the youth in placement when appropriate.
◾In collaboration with the Case Manager, conduct follow-up visits with the youth and alternative placement to ensure stability of the placement.
◾Documents activities in Florida Safe Families Network (FSFN) electronic client record system.
◾Assist in training, mentoring staff on practices and procedures related to Family Finding search and discovery.
◾Other duties as assigned.

Skills / Industry Qualifications Required:

Skills and Abilities:
◾Intermediate data entry skills
◾Proficiency in Microsoft Office Suite including Word, Outlook and Excel
◾Excellent interpersonal and communication skills
◾Strong time-management and organization skills
◾Ability to analyze, integrate and report data
◾Demonstrate cultural & linguistic competence & sensitivity to population served
◾Willing to travel
◾Ability to work flexible hours and on-call
◾Program development, evaluation, solid organizational skills, and public speaking
◾Strong written and oral communication skills
◾A reliable automobile with adequate insurance coverage

Degree Type/Area: Bachelor’s degree in Social Work, Human Services or a related field; Master’s Degree preferred. Minimum of 3 years of experience, State of Florida Child Welfare Certification required. Knowledge of funding & provider resources available in the county and surrounding areas. Familiar with resources/services that supports children with substance abuse, mental health, developmental disabilities and delinquency issues.

SPECIAL CONDITIONS:
◾Thorough Background Screening upon hire including FBI, FDLE, Local Law check, Employment Reference check, DMV check, Criminal Records check, Social Security Trace & Drug Screening.  FDLE check will be run at least every 5 years.
◾On-call assignments- YES
◾Travel- local: 15% per week, long-distance: 5% per month
