The Athletics Department will be using FamilyID to complete sports registration online. You can initially enter all children in your family account and then sign up students for individual sports before the start of athletic seasons and marching band.
A parent /guardian can create a student account by clicking https://www.familyid.com/voorhees-hs-athletics.
Create your secure FamilyID account by entering your family name, email address and password.
You will receive an email with a link to confirm your new account. (If you don’t see the email, check your span or junk folders)
***Family ID will not replace the physical examination form, which should still be turned into the health office on a yearly basis***

Each season: The following must be completed before the due dates listed above in order to participate.
Complete the hard copy Health History Update Questionaire and give to the school nurses.
Fill out the information requested on the FamilyID registration form before each season/activity. See table above for availability dates.
You will receive an email from FamilyID confirming your registration. Log on at any time to update your information and to check your registrations. For technical assistance 7 days per week, please call FamilyID at 888-800-5583 X1.
Once a year: Early submission of the physical exam is welcome!

Families can check for current physical exams and Medical Examiner clearance to participate at ID_participation5-16-14 . Current physicals are listed, blank space indicates new PE needed. Clearance to participate indicated by MD check. Health History Questionaire completion is also noted.
Physical examinations and Health History Update Questionaires can be returned to the box on the door of the health office if the nurses are not in. (Nurses have very limited hours in the summer.) They can also be faxed to 908-638-2188 or sent to the nurses’ email addresses below.
FINAL DUE DATES FOR PHYSICAL EXAMS AND HEALTH HISTORY UPDATE QUESTIONNAIRE:
July 24th – for fall athletic participation and marching band.
November 5th – for swimming
November 21st - for winter sports.
February 20th - for spring sports.
There will be at least a 24 hour waiting period for athletic participation for Family ID registration or physical examinations received after the due dates.
