The full complement of Home Care services include: physical, occupational and speech therapies, case management, palliative care, perinatal care, infusion therapy, medical social workers, skilled nursing and home health aides. W/E Leadership call -Support to staff - rotational responsibility Primary Function: .and. Major Responsibilities The Clinical Manager is responsible for the oversight of a multi-discipline team, which includes the planning, directing, organizing and leading the team to achieve best practice patient centered provision of care. The Clinical Manager supervises, supports, and educates staff at the point of service in the field. The Clinical Manager demonstrates evidence based leadership. Supports an efficient and accurate revenue cycle by enhancing capture of appropriate revenue and managing the flow of documentation within their department to minimize days in accounts receivable. Acts in a manner that is consistent with HealthEast s faith-based philosophy, values and business ethics. Makes decisions and takes actions that reflect patient service at the center of all we do. Oversees an interdisciplinary team which manages an active patient census of approximately 150-200 patients. Directs the assignment of care, clinical integrity of practice, and flow of documentation through the support of team members. Mentors and measures competency in the areas of: 1) clinical assessment, 2) competent OASIS data-gathering and direct interpretation, 3) time management toward achieving efficient and effective care delivery, 4) documentation skill-sets at the point of care, 5) team efficiency and operational support systems (Team Assistant and Medical Records staff). Builds and evaluates team members throughout the employment process; mentoring and holding clinicians accountable to established agency standards of practice, documentation, and productivity: Collaborates with Director of Quality Management, Performance Improvement Team and RN Clinical Educator regarding OASIS integrity, individual abilities and deficits in documentation. Maintains team CMI averages within benchmarked expectations, creating a culture of accountability for OASIS integrity. Holds staff accountable to the agency productivity standards and mentors staff in skill sets needed for effective time management and enhanced productivity capacity. Monitors individual and team performance and makes adjustments as needed to deliver results. Accurately evaluates the work of direct reports and provides prompt feed back, redirects efforts for improved results, and conducts timely formal performance appraisals. Completes OASIS integrity audits in the field as a part of performance improvement incorporating results into the performance appraisals. M
