BHT Responsibilities:

Includecollaboration/coordination of behavioral health services between ACTSand other involved agencies (DES, CPS, Adult and Juvenile Probation,school districts, advocates groups, etc.). Assist other Clinical Teammembers to support Client and their families in order to provide anadequate quality of life.

• ProvidesCase Management services for assigned clients, including coordinatingand monitoring the activities of the individual treatment team andassisting in developing, writing, and implementing overall goals andplans.

1. Guides theservices of Support personnel.

2. Responsiblefor clinical case load.

• Completionof Service Plans and Crisis Plans with all team members presents toprovide input and feedback.

• Releasesof Information will be completed for all outside agencies (JPO, DES,CPS, etc.) and other treatment team members identified by client.

• Completionof appropriate referrals for treatment services within agency and allother outside agencies as needed.

• Assistwith reengagement efforts, as needed (i.e. 3 telephone, mail, and/orhome visits followed by a certified letter) to ensure consumers areinvolved in treatment services.

• Performsclinical treatment of substance abuse and general mental healthconsumers for both children and adult.

• Completesclinical summary and ensures that complete consumer dischargesummaries and/or closures once services are completed.

• Consultswith community agencies and families to maintain coordination in thetreatment process.

• Completesthe assessment of customer needs for support in education, socialdevelopment and functioning and daily living (assessment every 6months for children and every year for adult clients)

• Willensure confidentiality for clients at all times with the exception ofDTS/DTO.

• Performsindividual and family counseling to enable consumers to identifyvocational strengths and needs while establishing goals and actionplans to reach those goals.

• Acts asthe Recovery Coach Manager and guides other Team members tofacilitate consumers with the appropriate level of Placement, ifneeded.

• Follow upon missed appointments, by phone or home visit.

• Updatedemographics every 6 months for assigned clients

• Mustattend first CFT/ART meeting to assist with treatment planning andattend subsequent CFT’s/ART’s when possible

• Completeand update CASII’s (Children) at intake, after first CFT every6 months, at discharge and as needed along with all other requiredpaperwork.

• AttendPsychiatric appointments with assigned clients

• Completetreatment plans and reviews for all assigned clients at intake,within 30 days, every 6 months, upon discharge when applicable and asneeded.

• Completeall required items according to checklists and CAPs per ACTS’Document Conformance Plan process.

• Contactall clients on caseload at a minimum at least one time monthly.Review caseloads to ensure who needs reengagement, closure follow upor additional services.
