**SUMMARY OF THE POSITION: **

Performs Condition Management services which includes educating members on self-management of a chronic condition in order to facilitate the best health possible

**ESSENTIAL DUTIES & RESPONSIBILITIES:**

ü Establishes and maintains effective working relationships with patients and their families, providers and payers.

ü Maintains confidentiality of received, provided, or processed information. Signs and adheres to the Xerox Confidentiality Agreement.

ü Identifies cases for condition management from information collected from patients, providers, and caregivers. Conducts a thorough assessment, develops and implements a condition management plan, including goals.

ü Manages case to conclusion, while maintaining contact with all individuals involved. Makes modifications as appropriate. Ensures that the goals identified in the condition management plan are met.

ü Uses established evidenced-based guidelines to evaluate treatment plans. Refers cases that do not meet evidence-based guidelines to the Medical Director.

ü Educate and assist patients in understanding and self-manage their chronic conditions within current medical practice.

ü Refers patients to appropriate resources to allow for positive outcomes relating to their chronic illness.

ü Identifies potential cost savings within a health plan’s coverage. Acts upon cost savings in instances when services would not be sacrificed by doing so.

ü Acts as primary contact with designated patients and providers.

ü Participates in quality improvement strategies by assessing individual and employer satisfaction with programs.

ü Participates in continuing education (internal and external providers) in order to maintain active case management certification, clinical and condition management expertise.

ü Participates in professional nursing organizations.

ü Assists in the implementation and adherence to policies, procedures, and new programs related to care management and quality improvement.

ü Obtains/maintains knowledge of Federal, State, and local funding and community resources available to patients and their families.

ü Performs all job duties and responsibilities within the scope of State & Federal laws, licensure/certification as well as within the ethics described in the CMSA’s Standards of Practice.

ü Adheres to URAC standards for programs the Company’s current accreditations.

ü Ability to read and interpret Summary Plan Documents relating to services requested.

ü Other duties and responsibilities as directed by supervisor.

**EXPERIENCE/ABILITY REQUIRED:**

ü Minimum of 3 years nursing experience in an acute care or relevant setting.

ü Proficiency in basic computer skills including Microsoft Windows and Works (others as identified by supervisor), and the ability to access and navigate the Internet to conduct research.

ü Effective communication skills—verbal, written, interpersonal, and phone, in order to interact with internal departments and external contacts.

ü Accurate spelling, grammar, and documentation preparation skills.

ü Ability to attend to numerous details under the stress of maintaining courteous, accurate and timely relations with a variety of individuals.

ü Ability to work independently, with minimal supervision.

**EDUCATION/LICENSURE/CERTIFICATION: **

ü Registered Nurse with active and unrestricted license to practice in the State of Wisconsin.

ü Certified Chronic Care Professional (CCP) (designation as soon as eligible to take the exam if not already upon hiring).
