Lucile Packard Children's Hospital Stanford is the heart and soul of Stanford Children’s Health. Nationally ranked and internationally recognized, our 311-bed hospital is devoted entirely to pediatrics and obstetrics. Our six centers of excellence provide comprehensive services and deep expertise in key obstetric and pediatric areas: brain & behavior, cancer, heart, pregnancy & newborn, pulmonary and transplant. We also provide an additional, wide range of services for babies, kids and pregnant moms.

Works closely with many departments and services including ancillary staff on the inpatient units and outpatient clinics, radiology, pathology as well as referring practitioners, families and patients.

Job Summary

This purpose of the Patient Care Coordinator is to work with the Nurse Practitioner/Physician Assistant for the Pediatric Liver/Small Bowel Transplant Program at LPCH in support of pre and post-transplant patient care management.

Essential Functions: Pre-Transplant

I.     Obtains outside records for pre-transplant referrals for liver and intestinal transplant patients. This includes obtaining all radiology, pathology, lab and pertinent records, patient histories, physical examination and additional records as needed.

Serves as expert and resource for pre-transplant care coordination.

Schedules pre-transplant evaluations per protocol.

Initiates appropriate consultation within the interdisciplinary team as needed.

Schedules any additional services for the evaluation as needed.

Prepares pre-transplant teaching materials and packets

Obtains laboratory and other diagnostic tests as needed for pre-transplant patients during the evaluation and during waitlist period.

Under the supervision of the NP/PA schedules treatments as necessary.

Coordinates pre-transplant evaluation and collects appropriate data required for the selection committee;

Documents in charts as necessary.

Attends patient selection meeting and documents plan of care as required

Attends weekly specialty service rounds.

Under the supervision of the NP/PA lists Patients according to UNOS and CMS guidelines.

Creates patient notification letters according to UNOS and CMS guidelines

Prepares weekly selection agenda as required.

Works with the NP/PA staff on improving clinic flow for transplant evaluations.

Act as a liaison between staff the patient/family and the liver/intestine transplant team and demonstrates a commitment to Pcares.

Essential/Specific Functions: Post-Transplant

Post-Transplant-Supports coordination of care for post liver and intestinal transplant patients.

Follows patients on an out-patient basis in conjunction with the transplant team and serves as a liaison to the patient and family and referring physician.

Ensures scheduled patients obtain test and services required as ordered.

Keeps the NP/PA staff current on a daily basis of scheduled patients of tests ordered and/or completed.

Assists with follow up of documentation of vaccination records as needed.

Schedules patients for clinic visits per protocol.

Prepares post-transplant teaching materials and packets.

Under the supervision of the NP/PA schedules treatments as necessary.

Documents in charts as necessary.

Obtains outside records for post-transplant patients as needed.

Facilitates scheduling for outreach clinics.

Prepares patient lists and required studies as up to date for outreach clinics.

Contacts referring practitioners for necessary patient information and keeps the practitioner current regarding completion on a daily basis.

Under the supervision of the NP/PA communicates specific lab results.

Works with the NP/PA staff on improving clinic flow for post-transplant patients.

Act as a liaison between staff the patient/family and the liver/intestine transplant team and demonstrates a commitment to Pcares.

Minimum Qualifications

Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.

Education: AA or AS in health related field or equivalent combination of relevant education and/or pertinent experience.

Experience: Three years’ experience in a medical, billing or health insurance office setting. Spanish speaking preferred.

License/Certification: None

Knowledge, Skills and Abilities

• Skilled in effective communications, customer service, problem solving, and managing multiple priorities.

• Knowledge of insurance eligibility, benefits, contract language, and medical terminology.

• Demonstrates knowledge of registration policies, FSC determinations and contract information.

• Accurately utilizes IDX insurance dictionaries to assign FSC.

• Demonstrates acceptance of patient and family regardless of race, gender, disease process, life-style, sexual orientation, religious or cultural beliefs.

• Follows policies, procedures and guidelines to manage patient care and other information.

• Ability to utilize appropriate manual and electronic systems.

• Ability to maintain confidentiality.

• Skill using keyboard and mouse for computer interface.

Preferred: Experience with registration.
