2. Context and Scope

HammondCare is an independent Christian charity that exists to improve quality of life for people in

need.

We specialise in aged and dementia care, palliative care, rehabilitation, mental health services for

older people, and other related health and aged care services. HammondCare seeks to embed

evidence based best practice in its services and we provide these services through home care,

sub-acute hospitals and residential aged care.

Bringing these health, hospital and aged care services together, HammondCare has been able to

develop innovative, flexible care models designed to serve people with complex health or aged

care needs, regardless of their circumstances.

The HammondAtHome(Community)

Care Worker delivers a range of in

-

home care services for

HammondCare clients in order to assist with their daily living. Our HammondAtHome Care

Workers work in clients homes

mostly alone, under the direct supervision and guidance of a case

manager. Our Care Workers are expected to deliver outstanding levels of client service and to

perform delegated duties to a high standard within the HammondCare mission and core values so

as to maximise client satisfaction

e ComPacks Program

ComPacks is a short term package of community support services aimed at helping people

manage a safe return home from hospital. A ComPacks package is available for up to six (6)

weeks following discharge from hospital.

Duties may include the following (refer Key Responsibilities for more detail);

Personal care needs

Medication supervision

Nutrition & hydration

Light house work

Socialisation

Shopping/appointments

In-home respite

Care workers are trained and supported in the use of a variety of mobility support

equipment, such as mechanical lifters, shower chairs, commodes, hospital beds, transfer belts,

as well as in the use of continence products, dressings, catheter/colostomy bags and peg

feeding. We are passionate about valuing the individual and having the opportunity to utilise the

time spent with the client as a positive time to engage them in an activity that will promote their

independence, encourage enjoyment and provide meaning. Activities may include, taking the

client for an outing to a special place of interest, reminiscing, reducing social isolation and the

like.

Key Responsibilities of the HammondAtHome (Community) Care Worker role

Provision of personal care services

to clients in their homes according to the Care Plan

which may include: Showering, bathing, sponging or bed sponge.

Grooming

All dressing and undressing of clients, hair care, and shaving.

Toileting – Including transfer to and from the toilet, loosening clothing, changing

continence pads, and assistance with toileting aids (e.g. catheters, colostomy bags

etc.).

Mobilising

assisting clients to transfer where clients have limited or no ability to

weight bear. Assisting clients with movement (e.g. bed to chair to bed), walking with

clients inside and outside the home, pushing and positioning clients in wheelchairs,

wheeled commode or shower chairs. Setting up and using mechanical lifters for clients

who are fully dependent. Also includes pressure care by assisting the client to

change positions.

Personal Hygiene

Assisting clients with skin care, nasal care and oral hygiene.

Medications

Assisting clients with the self-administration of medicines (liquids,

suspensions, tablets, etc.). Also supporting the administration of nose, ear and eye

drops, as well as nebulized medications. Giving medications from blister-packs and

monitoring blood glucose levels.

Meals and Eating

Cooking meals and assisting clients to eat a meal.

Household duties

may include, cleaning, vacuuming, dusting, washing of floors,

changing bed linen, washing clothes, hanging out washing.

Socialisation

taking clients outside the home (e.g. shopping, medical appointments

etc.) and assisting clients to move into and out of a motor vehicle. Includes lifting support

items into and out of motor vehicles (including wheelchairs, walking frames, shopping

bags, etc.)

Professional boundaries

maintaining appropriate professional boundaries to protect

our clients and ourselves. Focusing on meeting

the clients needs, not our

own personal

needs. Establishing a professional, rather than a personal relationship with our clients.

Protecting the privacy, dignity and rights of our clients.

Please note that the level of physical and emotional effort required by the care worker in each

task will vary with the care requirements and care plan for each client. This could range

from

minimal

assistance

to

standing

by in the event of

unsteadiness

through to

full

assistance,

where there are severely limited, uncontrollable movements or serious

comfort/health considerations. Some lifting or physically awkward movement is involved for

staff in caring for clients with limited or no mobility.

Administration and Quality

Provide continuity of care

by updating care activities and client status in clients notes

and ensure completion of all documentation.

Maintain confidentiality of client information at all times.

Maintain ongoing and effective communication with the Case Manager, including the

giving and receiving of relevant information.

Ensure immediate escalation to the Case Manager of any contentious issues in the care

of a client.

Follow through with directions and instructions from the Case Manager.

Develop a good working knowledge of all HammondCare Policies and Procedures,

particularly those relating to quality of care, and to work within those guidelines

while providing care services.

Accurately report kilometres, breaks and broken shifts using the smart phone.

Client and Staff Safety

Abide by all HammondCare Work Health & Safety Policies and Procedures,

including reporting of hazardous procedures and work areas and anything that has or

might compromise the care of the client or the health of the staff member.

Work cooperatively with Case Managers and team to resolve issues of concern.

Report all personal accidents or injuries, as well as any incidents or injuries

(no matter how minor) to the client.

Operate motor vehicle in a safe manner when driving to and from client locations,

particularly when transporting clients. Observe all necessary road rules.

Care Team membership

Work as a member of the HammondAtHome Team to provide continuity of care to our

clients, including assisting and training other staff as required.

Attend staff meetings and training sessions as required and be actively involved in

these sessions.

Recognise

the critical role of clients families and work supportively

with family

members, including ongoing monitoring of any difficult situations and escalation of

issues to the Case Manager where appropriate.

Please note that responsibilities may vary at the discretion of the Case Manager and in

consultation with the employee to meet changing organisational needs. The employee will be

notified of changes in their responsibilities; however, they will not be required to sign an

updated job description unless their role is significantly revised.
