mecwacare has provided care to the community for more than 55 years, offering residential aged care, community care, respite care, in-home care and support, disability and nursing services, providing optimal choice for people of all ages. We care for over 10,000 people each week across greater Melbourne through an integrated service network operated by more than 1100 employees and 250 volunteers.
We have an exciting opportunity to join our Home Nursing and Care Team as a Care Coordinator Consumer Directed Care Packages in a part time capacity 3 days a week for a 12 month contract. (22.8 hours per week)
This role is instrumental to this growth service in developing and managing home care packages for aged clients. Your responsibilities will include day to day management of a client case load, overseeing care planning and provision of flexible services to complex care clients within budget guidelines.
To be successful within this role will have to address the following selection criteria in the application:
Experience in assessment, case management and service delivery to aged clients living in the community
Well developed communication, planning, organisational and time management skills.
Knowledge of Home Care Packages andConsumer Directed Care principles
High level problem solving and analytical skills
Experience managing clients individual budgets
Ability to work independently and as part of a team.
Excellent computer skills, knowledge of Carelink Plus would be an advantage
Proven ability to establish effective and cooperative internal and external working relationships
Tertiary qualifications in Social or Health Science, Registered or Enrolled Nurses with current registration.
Additionally successful candidate will be required to have current drivers licence and satisfactory police check
