The Case Manager is a role within Disability SA and is accountable to the Team Manager for: Providing case management services to clients, families and carers with complex needs, including the assessment of client needs and the coordination of supports; identifying resources and services required by clients, and referring clients to relevant services; collaborating with other agencies and community groups to develop support networks for people with a disability and their families and carers.

Essential Minimum Qualifications: Appropriate tertiary qualifications in community services, social sciences, human services, health or related field.

Special Conditions: This is a temporary / term vacancy up to 2/12/2016. Must have a current and valid drivers licence. Some out of hours work including intrastate travel may be required. Successful applicant will be required to satisfactorily complete a Background Screening and National Criminal History Check (NCHRC) prior to being employed and every three years.

Role Status: Term up to 2/12/2016

Remuneration: $58,555 – $71,864 (PO1)
