2. Context and Scope
HammondCare is an independent Christian charity that exists to improve quality of life for people in
need.
We specialise in aged and dementia care, palliative care, rehabilitation, mental health services for
older people, and other related health and aged care services. HammondCare seeks to embed
evidence based best practice in its services and we provide these services through home care,
sub-acute hospitals and residential aged care.
Bringing these health, hospital and aged care services together, HammondCare has been able to
develop innovative, flexible care models designed to serve people with complex health or aged
care needs, regardless of their circumstances.

Summary of Position
The SBRT initiative is a federally funded initiative that provides a mobile workforce of clinical
experts who will provide timely and expert advice to residential aged care providers that request
assistance with addressing the needs of people with very severe and extreme behavioural issues
and Behavioural and Psychological Symptoms of Dementia (BPSD).
The SBRT team consists of nursing, allied health and medical dementia specialists supporting staff
in residential care settings with assessment, advice, medium and long term interventions.
Operating from 7am to 7 pm, 7 days per week (including public holidays,) the SBRT utilises a
multidisciplinary dementia specialist model to respond to cases referred by state based DBMAS
teams. The core team of consultants, supported by a network of metropolitan, regional and rural
associate staff, will draw on medical and psychology expertise to ensure complex care
management needs are met.
The role of
Consultant, SBRT
works within the Mission and Mission in Action of HammondCare to
ensure that the SBRT service is delivered each day to meet the needs of individual clients.

of stakeholders.
Travel, to locations across the state to
assess and support the care of individuals
with severe and persistent behaviours of
concern.
Participate in appropriate training with both
the Hammond College team and other
identified programs.
In conjunction with the SBRT Manager,
utilise an agreed set of assessment tools
and case management techniques to
support client needs.
Meet regularly with the SBRT Manager to
review the operational and strategic
performance of the service, problem solve
on more challenging cases, seek case
management resources as required.
Escalate inappropriately referred cases to
the manager for discussion with referring
DBMAS.
