POSITION SUMMARY

Responsible for providing leadership for all components of the District Stroke Program at Mackenzie Health. The Coordinator District Stroke Program will work with local partners to develop and sustain an integrated regional program which coordinates the infrastructure required for a full continuum of stroke care. Coordinator will work collaboratively with Mackenzie Health Leadership, Central East Stroke Network, Central LHIN partners: North York General Hospital, Humber River Hospital, Markham Stouffville Hospital, and Southlake Regional Health Centre to ensure a full continuum of care for stroke patients is developed and sustained. Coordinator will be responsible for sustaining a positive work environment which supports employee learning and growth and evidenced-based practice. Responsible and accountable for the coordination of human and financial resources required to provide quality patient care across the continuum.

PRIMARY RESPONSIBILITIES

Responsible for overseeing the Memorandum of Understanding with York EMS for the redirection of Stroke patients to Mackenzie Health for consideration of tpA eligibility.

Maintain, monitor and interpret program reports and statistical data (e.g. clinical indicators, payroll, financial) and provide updates.

Consult with team members to determine budget requirements and work with Centre Leadership team to develop the budget.

Lead the development of business cases and impact analyses for new projects and programs in collaboration with the Centre Leadership team.

Participate in the hiring process of new staff by managing the interviewing and selection process for MH Stroke Prevention Clinic (SPC).

Ensure the orientation of new staff in collaboration with the Program/Centre leadership team.

Facilitates and implements accreditation standards to meet Accreditation Canada Stroke Distinction requirements.

Participate in the performance management of staff in MH SPC.

Support staff development and continuing education opportunities and works with Regional Educational Coordinator.

Accountable for the development and implementation of administrative and clinical policies and procedures across region.

Actively participates and provides leadership on internal and external committees as appropriate including District Stroke Centre Quality Care Committee, Central East Stroke Committees, LHIN Stroke Planning and Care Council and Ontario Stroke Coordinator and Regional Stroke Committees and planning sessions.

Chair, Co-Chair committees as required.

Creates and maintains a safe and professional environment for efficient delivery of quality patient-focused care.

Communicate with other healthcare professionals and interdisciplinary teams

Serves as an interdisciplinary resource.

Coordinate internal and external resources to facilitate patient care.

Provide exemplary leadership to the interdisciplinary team.

Clearly articulate and model a patient-focused philosophy of care for stroke patients and survivors.

Advocate for patients, survivors and staff.

Provides leadership with in the stroke service related to quality improvement initiatives and other special projects (e.g. task forces, workload measurement, staffing, education and orientation of students).

Assign appropriate activities to staff and volunteers while providing effective supervision and leadership.

Practice in accordance within specific professional standards.

Seek help and accepts guidance when a gap in competence is identified.

Maintain own clinical and professional competency.

Maintain membership in professional role-related organizations.

Commitment to lifelong learning and continually seeks ongoing education.

QUALIFICATIONS

Registered Nurse in good standing with the College of Nurses of Ontario.

Baccalaureate in Nursing with Master’s degree (preferred).

Minimum 5 years recent clinical experience required in a medical/stroke service.

Experience in a leadership role in a health care environment.

Experience with stroke management and familiar with continuum of stroke care.

Previous project management experience in a hospital/clinical environment.

Exemplary leadership, interpersonal and communication skills.

Demonstrated clinical practice knowledge.

Previous experience in leadership, research, program development and evaluation.

Ability to discern relevant information to make effective and timely decisions regarding the organization and functioning of the patient care continuum/services.

Demonstrated ability to create and coordinate multidisciplinary professional teams.

Demonstrated conflict resolution skills.

Proven ability to work independently, take initiative and prioritize work.

Familiar with the professional standards of practice for all staff within the continuum of stroke care.

Ability to assess, interpret and project budgetary needs.

Proven ability to work within a computerized environment using Word, Excel, PowerPoint, Access and Outlook as well as demonstrated knowledge of data collection, use of data bases, data analysis, budgeting and evaluation.

Proven ability to facilitate ongoing staff development through mentoring, coaching and counseling skills.

Excellent written and verbal English language skills.

Valid Ontario Driver’s License as travel will be required.

Proven attendance record.
