A key leadership role in implementing a transformational approach to integrating mental health and addiction service delivery

Canadian Mental Health Association (CMHA) York Region is working with the Central Local Health Integration Network (Central LHIN) to spearhead a collaborative, new and innovative approach to implement an integrated service delivery model for mental health and addiction services. At the heart of this exciting project is a commitment to engage persons with lived experience to inform service redesign and quality improvement initiatives.

CMHA York Region was one of a number of key Health Service Providers that collaborated with the Central LHIN to develop a multiyear action plan to address the mental health and addictions service needs of residents of York Region.

The Central LHIN works with local health service providers and community partners to plan fund and integrate local health care, improving access and the patient experience. The goal is to develop systems of care that will be strong and sustainable both today, and into the future.

The operationalization of the Central LHIN Mental Health and Addictions Supports within Housing Action Plan for the Region of York will require implementation of multiple initiatives across a number of organizations in a standardized and coordinated approach.

This position provides a key leadership role in a three and a half year project to operationalize the Central LHIN Mental Health and Addictions Supports within Housing Action Plan for York Region, which also aligns with the CMHA’s commitment to improving the wellness of persons with mental health and addictions and community integration. The Director of Service Transformation will be a fixed term contract for the duration of the project.

Reporting to the CMHA York Region and with a very close working relationship with the Central LHIN, the Director of Service Transformation will have responsibility for implementation of the initiatives in the Central LHIN Mental Health and Addictions Supports within Housing Action Plan for York Region supported by a Service Coordination Council, which is being established to both collaborate on implementation as well as provide guidance on “how” to implement.  Implementation of the Action Plan will enhance the wellness of persons with mental health and substance abuse conditions by integrating/coordinating care at the local level. The Council is currently being established and will include a minimum of 30% people with lived experience of mental illness and/or addiction and their family members, as well as representation from health care, social services and the municipality.

As an accomplished transformational leader and a change management expert with a passion for the  long-term impact of effective mental health and addiction services and an ability to inspire it in others, you will provide support and direction to the Service Coordination Council and the Co-Chairs. As a systems thinker and collaborator, you will establish and maintain highly effective relationships with mental health and addictions providers, community agencies and key strategic partners across York Region. In addition to your advanced knowledge of evidence-based practices in community mental health and addictions service systems, and your proven experience in project management, change management and evaluation, you will have excellent interpersonal and conflict-resolution skills and demonstrate an ability to synthesize health service delivery systems and the social determinants of health for the communities served. You will also have the ability to elicit new ideas and build support for developing innovative solutions in a strategic context.
