Qualifications

PRN working day shift, primarily weekends

The Hospice Staff Nurse coordinates all aspects of health care in the home for patients under the direct orders of a licensed/credentialed physician. Demonstrates strong problem solving skills, the ability to work autonomously and as a team member, has strong teaching skills and the ability to work within a multidisciplinary system. Performs competent procedures and quality assessments. Provides excellent patient care and meets the documentation requirements of hospice services regulations and credentialing organizations. Participates in the agency Quality Assessment and Performance Improvement (QAPI) program.

Follows all established organization and department policies, procedures, objectives and quality assessment. Responsible for assisting in maintaining a safe working environment throughout the organization by adhering to all safety management policies and procedures which reflect the mission and philosophy of the Ascension Health System. Attends all mandatory in-service programs and maintains professional and education requirements.

Education Required
Graduate of professional school of nursing.

Education Preferred
Bachelor's degree in Nursing or other applicable health care field

Registration, Certification and/or Licensure Required
Current RN license in State of Indiana
Current CPR certification
Negative Indiana criminal history check
Valid Indiana driver's license with acceptable driving record to be maintained during employment with Saint John's Health System

Registration, Certification and / or Licensure Preferred
Certified Hospice Palliative Nurse (CHPN) certified

Experience Required
2 years General Medical/Surgical, Oncology or ICU nursing in a hospital or 2 years Home Care or ECF experience.

Experience Preferred
1 year in a Hospice or Palliative Care setting

Skills, Knowledge and Abilities Required
Possesses excellent oral/written communication, documentation and human relation skills.
Ability to coordinate patient's care with interdisciplinary teams and/or other community agencies.
Manages time to meet needs of scheduled visits and/or unexpected crisis.
Basic computer skills for patient visit documentation.

Working/Environmental Conditions:

Working in a private home
May make home visits at night
Requires judgment/action which could result in death of patient (RN¿s)
Noises may be sufficient to cause distraction
May be exposed to extremes of heat and cold in all weather conditions
Exposed to infection from disease-bearing specimens
May be exposed to infectious and contagious diseases
Regularly exposed to the risk of bloodborne diseases
Exposed to hazardous body fluids and wastes
Subject to electrical energy hazards
Exposed to hazards from electrical/mechanical/power equipment
May be exposed to bio-hazardous, radioactive substances
Exposed to odorous chemicals and specimens
Exposed to housekeeping/cleaning agents/chemicals
May be exposed to toxic chemicals
Contact with patients under wide variety of circumstances
Handles emergency or crisis situations
Occasionally subjected to irregular hours and schedule changes
Travel required ¿ must provide own transportation and current auto insurance

Physical Demands
Must be able to maneuver in and out of an automobile to gain access to patient¿s home.
Heavy physical effort; must be able to assist with lifting, positioning and transferring patients up to 50 pounds.
