This position is located in the IHS Bemidji Area, Cass Lake Service Unit; Health Information Management/Medical Record Department. The primary purpose of this position is to apply appropriate disease classification, procedure and supply codes to episodes of care for primarily the Inpatient Department.
* Assigns and sequences a variety of codes including but not limited to ICD, CPT, and HCPCS codes based on the medical record analysis. Assures the final diagnosis and operative procedures as documented by the provider are valid and complete. When multiple diagnoses and procedures are listed, assures the procedure is related to the proper diagnosis. Makes the final determination that medico-legal requirements of the record is complete, accurate and reflects sufficient data to justify the diagnosis and warrant treatment and end results. Reviews medical record for compliance with established third party reimbursement agencies and special screening criteria.
* Performs quantitative analysis by evaluating the record for documentation consistency and accuracy. Assembles medical record into the proper sequence in accordance with institutional chart regulations. Monitors and records discrepancies from PCC record review and returns forms to providers for correction.
* Analyzes provider documentation to assure the appropriate Evaluation and Management (E&M;) levels are assigned using the correct CPT code.
* Works with coding staff to improve coding accuracy through periodic coding audits, chart reviews, and training. Collaborates with the Business Office and other departments to improve billing and collection, documentation and other processes through the facility. Provides ongoing education, updates and briefings for the medical staff, business office staff, and other health care providers on the changing coding conventions, rules, regulations and guidelines.
* Compiles and retrieves medical records data for audits, performance improvement studies, research, patient care evaluation activities, reports, and Service Unit Compliance.
* Assists in development and modification of facility coding policies and procedures.
* Maintains record confidentiality in accordance with the Privacy Act of 1974, Health Insurance and Portability Act, Freedom of Information Act and other pertinent federal regulations.

**BASIC REQUIREMENT(S):** Experience and Education Requirements: GS-07: One (1) year of specialized experience equivalent to at least GS-06 grade level.
**SPECIALIZED EXPERIENCE:** Experience that equipped the applicant with the particular knowledge, skills, and abilities (KSA''s) to perform successfully the duties of the position, and that is typically in or related to the position to be filled. To be creditable, specialized experience must have been equivalent to at least the next lower grade level.
**SPECIALIZED EXPERIENCE DEFINED:** For GS-07: Comprehensive knowledge of the structure of physiology and major anatomical system and major disease processes of each. Extensive knowledge of medical record classification systems and references such as the ICD-9-CM, CPT, and HCPCS coding systems, Diagnostics Related Groups (DRG’s system), Physicians’ Desk Reference (PDR) medical dictionaries, manuals, textbooks and glossaries to accurately code and classify diseases, operations and other medical procedures. Extensive knowledge in the medico-legal aspects of medical record systems. Knowledge of the Privacy Act of 1974 in regards to the incumbents’ rights, responsibilities and possible penalties when patient confidentiality is violated.
Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
