This position will cover DC and Southern Maryland Areas. We prefer applicants from these **__Specific towns:__** Bowie, Upper Marlboro, Silver Spring, Hyattsville, Gaithersburg and Rockville.
**JOB SUMMARY**
Provides, coordinates, teaches, and/or supervises care for a group of patients usually in the setting of the patient’s home. Employs the appropriate discipline specific process to develop and implement plans of care for patients requiring in-home health and support services, under physician orders. In addition to specific and skilled treatments and procedures, the clinician:- evaluation and assessment of patients,- care provision, health, and/or developmental teaching to patients and care givers- preventive health, care, and/or developmental screening as appropriate,- referral and coordination of services in the agency and the community,- and advocacy for the patient in obtaining / attaining health-related goals clinician ensures adequate service provision, including scheduling of visits, based on patients’ identified needs and priorities for care. Complies with all Home Care policies and procedures and regulatory requirements. Assists in daily activities of the home health agency administration, orienting / precepting of new employees, and utilization review / management functions as requested and appropriate.
**JOB REQUIREMENTS**
**Education:** Accredited Nursing School graduate; BSN preferred
**Knowledge:** Current application of nursing / therapy theory and skills for the evaluation, identification, and treatment of health needs of patients requiring home health care;Ability to provide skilled evaluation and care reflecting knowledge of State of Maryland Scope of Practice Acts as it applies to nursing; Knowledge of the services provided by other health care professionals in order to make timely and appropriate referrals for service; Understanding and ability to apply principles of teaching and learning to patients with varied abilities and needs.
**Skills:** Ability to communicate clearly and effectively, verbally and in writing, with varied persons including patients, families, home care staff, physicians, and administrators; Ability to exercise independent, sound judgment and to function efficiently, effectively, and independently; Organized in approach to work accomplishment and meeting time requirements; Ability to understand and comply with Agency and regulatory standards of care; Discipline appropriate physical / cognitive / neuro-developmental assessment skills (age appropriate as applicable); Phlebotomy; Ability to accept, work and effectively teach & communicate with individuals and families with varied social and cultural backgrounds without being judgmental; Safe, competent, innovative, and therapeutic hands on clinical skills required; A high degree of flexibility and the ability to establish priorities; Ability to supervise other staff as delegated & appropriate per scope of practice (LPN / HHA / PTA / COTA); Ability to communicate and function as a member of a multi-disciplinary team to enhance collaboration and problem solving.
**Licensure/Certification:** Current licensure as RN in Maryland; Current CPR certification; Valid Maryland drivers’ license and current auto insurance, working vehicle; agree to comply with Maryland law for mandatory seat belt use
