A greatSkilled Nursing Facility (SNF)is looking for an experienced or newly graduatedand hard-working Program Manager to join their growing team!
The Program Manager ensures that the highest standard of rehabilitation services is provided and maintained by your staff. The Program Manager will interview, hire and supervise personnel and consultants providing service within facility or home care contracts. He/she assumes responsibility for daily staffing and utilization with input from treating therapists, consideration of clinical requirements, and recommendations from the Clinical Specialist. The Program Manager also ensures that the clinical management of the rehab department, including oversight related to case management, quality improvement, care planning, clinical utilization, and patient identification is overseen in addition to being responsible for building a department budget, monitoring revenue and expense and reporting. The Program Manager will also educate patient diagnosed with a variety of illnesses to better understand and manage their conditions as well as use discharge planning skills to promote independence, maximize function and return patients to their homes. He/she will assume responsibility for insuring departmental compliance with all regulatory and practice act requirements.
Requirements include:
Must have graduated from an accredited school.
Current state license as an Occupational Therapist/ OT, Physical Therapist / PT, Speech Language Pathologist / SLP, Occupational Therapy Assistant / COTA or Physical Therapy Assistant /PTAor proactively in the process of the application process for current state licensure as an OT, PT, SLP, COTA or PTA.
Current CPR certification.
Collaborate with all disciplines to plan andevaluate team goals for each patient.
Provide skilled therapy services / interventions in accordance with physician orders.
Advises, consults and instructs members of the multi-disciplinary therapy staff in specific physical, occupational and speech therapy modalities and interventions.
Responsible for the overall administration, clinical planning, development and operations of the rehab program.
