The All-on-4 Dental Implant Procedure - A Brief History
The All-on-4 Dental Implant Procedure or The All-ON-4® Treatment Concept is a procedure for the total rehabilitation of the edentulous patient or for patients with badly broken down teeth, decayed teeth or compromised teeth due to gum disease, The concept was developed, institutionalized and systematically analyzed in the 1990’s by Portuguese Dentist Paulo Maló. It consists of the rehabilitation of the edentulous maxilla and mandible with fixed prosthesis by placing four implants in the anterior maxilla, where bone density is higher. The four implants support a fixed prosthesis with 12 to 14 teeth. In 2011, Nobel Biocare Services AG trademarked ALL-ON-4® in the United States.

The All-on-4 Dental Implant Procedure provides a, screw-retained, same-day replacement for the entire upper and / or lower set of teeth. The All-on-4 Dental Implant procedure is best for patients with significant tooth loss or decay and for people whose bone loss in the jaw area prevents them from getting dental implants. The All-on-4® treatment concept consists in establishing a fully customized denture (bridge) based only on four titanium implants. These implants act as “pillars” to the bridge that supports the prosthetic teeth. The invented process takes advantage of existing bone, even if scarce, avoiding having to use bone grafts.
The posterior implants are substantially angled at approximately 45 degrees to avoid compromising the sinus cavity in the upper jaw and the nerve canal in the jaw. The All-on-4® treatment concept owes its name to the very nature of the method, which consists of applying a dental prosthesis with at least twelve teeth (bridge) fixed in the jaw, based only on four titanium implants.

Common Age Tested Engineering Principles
The concept centers on common engineering principles. The concept derives is strength by the angulation of the implants, which act as an inverted truss. In engineering, a truss is a structure comprising triangular units constructed with straight members whose ends are connected at joints referred to as nodes. External forces and reactions to those forces are considered to act only at the nodes and result in forces in the members which are either tensile or compressive forces. Moments (torques) are explicitly excluded because, and only because, all the joints in a truss are treated as revolute.
Another simpler way of illustrating this principle is evident in the common picture hanger. The strength of the device comes from the fact that the nail is driven at an angle.
The principle of angles has been evident since man began building structures.
A common principle in woodworking is: While driving a nail straight will hold nicely, for a better grip, drive in the nails at an angle. Nails that are hammered at an angle grip the wood better and provide greater strength and durability.
The implants used for the ALL-ON-4® Treatment Concept, NobelActive were originally designed by AlphaBio Implant company in Isreal. Nobel Biocare Services AG subsequently brought AlphaBio.
