Purpose:

To investigate three-dimensionally the locational relationship between retinal, retinal pigment epithelial (RPE), and choroidal lesions in eyes with central serous chorioretinopathy.

Methods:

14 eyes of 13 subjects diagnosed as central serous chorioretinipathy (CSC) underwent fundus color photography, fluorescein angiography (FA), indocyanine green angiography (IA), and en face OCT (Avanti RTVue XR, Optovue). Images were analyzed three-dimensionally, using superimposition method.

Results:

Three eye had acute CSC, 4 eyes had chronic CSC, and 7 eyes had recurrent CSC. Seven eyes had active CSC, showing focal leakage on FA images (3 acute, 2 chronic, and 2 recurrent). En face OCT revealed subretinal fluid (SRF) in 10 eyes, RPE hyperreflectivity in 14 eyes, RPE detachment (PED) in 9 eyes, and dilated choroidal vessels in 14 eyes. Dilated choroidal vessels were also observed on IA images. The location of dilated choroidal vessels were overlapped with those of SRF or hyperreflective RPE lesions. In addition, focally expanded lesions of dilated large choroidal vessels were found on en face OCT images in all eyes (14/14), which were overlapped in location with active leakage points in 100% (7/7), with hyperreflective RPE lesions in 100% (14/14), with PED in 77.8% (7/9), and with SRF in 90% (9/10).

Conclusions:

Choroidal changes showed high locational relationship with retinal and retinal pigment epithelial changes, and might be associated with pathogenesis of RPE or retinal abnormalities in central serous chorioretinopathy.
