1st Prof. Pharm. D (5 years Programme) at Department of Pharmacy, Shaheed Mohtarma Benazir Bhutto Medical University, Larkana, Sindh.
ELIGIBILITY
A. ACADEMICS:
> Minimum Qualification: Higher Secondary Certificate-!! (HSC-ll)/A-Level Pre-Medical (Biology, Chemistry, Physics) from any of the Educational Board in Pakistan or Equivalence Certificate from IBCC Islamabad in at least “B” Grade (60% Marks) of the year 2012,2013 and 2014.
B. DOMICILE AND PRC FORM “C”
> Applicants must have the domicile on P-l Form and PRC on Form “C” along with domicile P-l of Father/Mother/Guardian on P-l Form of the same district tagged to SMBBMU, Larkana, applying for Pharm. D Programme viz General Merit, UEAP (Local and Overseas Pakistanis).
C. SUBMISSION OF APPLICATION FORM:
> Prospectus along with Application Form is available at Admission Office of SMBBMU, @ Chandka Medical College, Larkana with effect from 22nd April 2015.
> Original receipt of payment of Rs. 1500/= (Rupees One Thousand Five Hundred only) as entry test fee (non refundable) in the name of Vice Chancellor SMBBMU, Larkana in account number 0259385831000344, paid in any branch of MCB Bank in Sindh. Write name of Candidate with Father’s name and district on back of the receipt.
> Original completely filled Application Form along with one powdered Photostat copy of required Documents duly attested by 18 or above Grade Officer.
