Prizes:
One (1) Grand Prize: one (1) travel package to Westlake Village, California for winner and guest. Travel must be taken within six (6) months of being awarded or prize will be forfeited in its entirety without compensation of any kind. Sponsor will determine airline and flight itinerary in its sole discretion. No refund or compensation will be made in the event of the cancellation or delay of any flight. Travel and accommodations are subject to availability, blackout dates, advanced notice requirements and other restrictions. Travel is subject to the terms and conditions set forth in this Contest, and those set forth by the Sponsor’s airline carrier of choice as detailed in the passenger ticket contract. If winner lives within reasonable driving distance of Westlake Village, CA (as determined by Sponsor in its sole discretion): (a) winner will not receive air travel element of Grand Prize and no compensation or substitute prize will be awarded in lieu of same and (b) winner may choose to use his/her own vehicle instead of car rental element of Grand Prize in which case he/she will instead receive $400 pre-paid Visa gift card as set forth below. All expenses and incidental travel costs not expressly stated in the package description above, including but not limited to incidentals, gratuities, passenger tariffs or duties, airline fees, surcharges, airport fees, service charges or facility charges, personal charges at lodging, security fees, taxes or other expenses are the responsibility solely of winner. Winner is solely responsible for obtaining valid government issued identification and any other documents necessary for domestic travel. Travel restrictions, conditions and limitations may apply. Travel companion of winner must be eighteen (18) years of age or older and must sign and return a liability release and a publicity release (where lawful), prior to issuance of travel documents. If winner chooses to accept Grand Prize without a travel companion, the Grand Prize will be awarded in increments appropriate for one traveler (i.e., the winner) and no compensation or substitute prize will be awarded. Sponsor will not replace any lost, mutilated, or stolen tickets, travel vouchers or certificates. Winner will not receive frequent flyer miles, hotel awards or other perquisites in conjunction with prize travel. Total ARV: $6,015. Actual value may vary based on airfare fluctuations and distance between departure and destination. Winner will not receive difference between actual and approximate retail value.

For car rental portion of prize, qualified drivers must be at least 25 years of age, have had a valid drivers’ license for at least one (1) year and have a valid major credit card in his/her name to cover the insurance deductibles. Charges not included at the time of booking include Collision Damage Waiver, Personal Accident Insurance, Theft Protection Coverage, gasoline, applicable taxes for optional services, local taxes, airport service charges, additional drop charges, concession recoupment fees and overstay fees. These fees will be payable by winner and winner is responsible to pay these charges directly to the rental car provider. Car rental companies will not make partial refunds or unused services to apply any portion of the rental to charges for gasoline, insurance or damages. If neither winner nor guest are at least 25 years of age (or fail to meet any of the other conditions of rental car portion of prize), rental car portion of prize will be substituted in the form of a $400 Visa** pre-paid gift card (subject to terms/conditions indicated thereon, which may include expiration date) and winner/guest will solely be responsible for their transportation during Trip.

Additional requirements and waivers/releases may apply to acceptance and use of Taste of Wellness Package element of the Grand Prize. This element of the Grand Prize (which includes health and wellness counseling and treatments) is being provided solely by California Health and Longevity Institute.

If purchase price of DOLE Bananas exceeds $2.00 or purchase price of any DOLE Packaged Salad, Fresh Pack Vegetable, Berries exceeds $3.99, winner will be solely responsible for the difference in price. Maximum ARV of each prize, $225.76. Total ARV $3,160.64. Each coupon must be used separately. Coupon subject to additional terms/conditions stated thereon. If purchase price of DOLE Packaged Salad, Fresh Pack Vegetables, or Berries exceeds $3.99, winner will be solely responsible for the difference in price. Maximum ARV of each prize, $105.76.
