### DUTIES:

**PLEASE READ THE ENTIRE JOB ANNOUNCEMENT BEFORE APPLYING**

This Clinical Pharmacist position is located within Pharmacy Service at the Dayton VA Medical Center, Dayton, Ohio and supports the distribution and monitoring of medication within this facility. The pharmacist will directly and indirectly give guidance and directions to technicians in the course of patient care while providing pharmacy services at this institution. Clinical Pharmacists will assist in training and orientation of other pharmacists and non-professional staff; promote primary goals and objectives; and will work in any area of pharmacy assigned on regular and irregular tour of duty. The Clinical Pharmacist will interact with other members of the staff in a responsible manner to ensure the well being of patients while rendering quality care. The position requires knowledge of and active participation in all of the duties and responsibilities of a Clinical Pharmacist in Pharmacy of Pharmacy Service. Performs other related duties as assigned by the immediate Pharmacy Supervisor/Chief, Pharmacy Service. Principle duties and responsibilities include but are now limited to the following:

* Provides advanced pharmacology and therapeutics consultation to all medical center staff.
* Obtains medication histories from patients, when appropriate, and summarize the significant findings. These findings are documented in the computerized patient record.
* Identifies any drug-induced problems, including adverse drug reactions, which may be affecting the patient or contributing to the need for hospitalization and acts to achieve their resolution. All adverse drug events are reported in accordance with Medical Center Policies.
* Develops individualized dosing regimens, based on the pharmacokinetics of the drug(s) employed and available lab data to make appropriate recommendations to the patient's provider.
* Interviews patients to determine medication history for potential drug interactions, therapeutic duplications, efficacy of current medication regimens, compliance patterns and/or drug reactions and acts to resolve any problems.
* Ensures that all Federal Laws and Regulations are met regarding the storing, dispensing, and documentation of Controlled Substances.
* Provides consultation to physicians, nurses and other health care professionals concerning drug therapy.
* Evaluates patients' medication and the patients' ability to understand medication instructions, while providing oral and written counseling to clarify or reinforce the patients understanding.
* Maintains confidentiality of patient/employee information, electronic and print.
* Provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate.
* Provides recommendations to prescribers on alternative therapies identified in the medication profile review process.

**Work Schedule:**** Full-time tour of duty (primarily between the hours of 7:00am - 6:00pm) Sunday thru Saturday. May include varying days, evenings and nights with rotating weekends.**

### QUALIFICATIONS REQUIRED:

**Basic Requirements:**

* U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

* Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.

* Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.

* English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

**Grade Determinations:** In addition to the basic requirements for employment in paragraph 2, the following criteria must be met when determining the grade of candidates.

**GS-11 Pharmacist**

**Experience, Education, and Licensure.** None beyond the basic requirements.

**Assignment.** Pharmacists at this grade level serve in a developmental capacity.

**GS-12 Clinical Pharmacist**

**Experience or Education.** In addition to the basic requirements, candidates must meet one of the following:

1. 1 year of experience equivalent to the next lower grade level, or

2. Completion of an ACPE-accredited Pharm.D. program.

**Assignment.** A pharmacist in the assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN0, and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of knowledge, skills and abilities (KSAs):

1. Knowledge of professional pharmacy practice.

2. Ability to communicate orally and in writing to both patients and health care staff.

3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.

4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
