### JOB SUMMARY:

This Clinical Pharmacist - Inpatient position is located within Pharmacy Service at the Dayton VA Medical Center, Dayton, Ohio and supports the distribution and monitoring of medication within this facility. The pharmacist will directly and indirectly give guidance and directions to technicians in the course of patient care while providing pharmacy services at this institution. Clinical Pharmacists will assist in training and orientation of other pharmacists and non-professional staff; promote primary goals and objectives; and will work in any area of pharmacy assigned on regular and irregular tour of duty. The Clinical Pharmacist will interact with other members of the staff in a responsible manner to ensure the well being of patients while rendering quality care. The position requires knowledge of and active participation in all of the duties and responsibilities of a Clinical Pharmacist within the Inpatient Pharmacy of Pharmacy Service. Performs other related duties as assigned by the immediate Pharmacy Supervisor/Chief, Pharmacy Service.

### DUTIES:

A Pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, non-formulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Principle duties and responsibilities include but are now limited to the following:

* Evaluates physician's orders and appropriateness based on medication profiles and known facts.
* Dispenses medication in accordance with dosage requirements and fully capable of functioning in various dispensing activities such as Unit Dose, IV Admixtures, Outpatient, and Narcotic vault.
* Provides quality control analysis for dispensing activities of automated dispensing equipment or pharmacy technicians.
* Assures that prescription/orders for controlled substances, I.V. admixtures, Unit Dose, and investigational drugs are filled in an accurate, thorough, and timely manner, in accordance with Service policy and procedures.
* Provides reliable pharmaceutical information to health care providers, always making certain to "follow-up" if the desired information is not immediately available.
* Ensures that all Federal Laws and Regulations are met regarding the storing, dispensing, and documentation of Controlled Substances.
* Provides orientation and guidance for new pharmacists, technicians, students etc., to ensure that Pharmacy and Medical Center procedures and techniques are shared and learned to the necessary degree by all employees.
* Monitor and track data as requested by the Pharmacy Supervisor. Will provide timely reports as requested.
* Develops and conducts educational programs on drug therapy for physicians, nurses, pharmacists, pharmacy students and other health care professionals.
* Maintains current competencies, continuing educational requirements, license requirements and expands professional abilities to satisfy the demand for enlarging personal role in caring for assigned patients.
* Incumbent possesses or develops an understanding and sensitivity of the individual needs, beliefs, and/or expectations of patients with various backgrounds.
* Protects printed and electronic files containing sensitive date in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, Federal regulations, VA statues and policy.

**Work Schedule:**** Full-time tour of duty, Monday - Friday, 1:30pm to 10:00pm**

### QUALIFICATIONS REQUIRED:

**Basic Requirements:**

* U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

* Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.

* Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.

* English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

**Grade Determinations:** In addition to the basic requirements for employment in paragraph 2, the following criteria must be met when determining the grade of candidates.

**GS-11 Pharmacist**

**Experience, Education, and Licensure.** None beyond the basic requirements.

**Assignment.** Pharmacists at this grade level serve in a developmental capacity.

**GS-12 Clinical Pharmacist**

**Experience or Education.** In addition to the basic requirements, candidates must meet one of the following:

1. 1 year of experience equivalent to the next lower grade level, or

2. Completion of an ACPE-accredited Pharm.D. program.

**Assignment.** A pharmacist in the assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN0, and national policies and regulations. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.

4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
