BUFFALO WILD WINGS FANTASY FOOTBALL FRENZY – OFFICIAL RULES
NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN. A PURCHASE OR PAYMENT WILL NOT INCREASE YOUR CHANCE OF WINNING.
1. HOW TO ENTER
a. These rules govern the Buffalo Wild Wings Fantasy Football Frenzy promotion being conducted by WSCR 670 (the “Station”) beginning on Monday, July 30th, 2012 at 12:00pm Central Time (“CT”) and ending on Wednesday, September 5th, 2012 at 11:00pm Central Time (“CT”).
b. To register a team into the Buffalo Wild Wings Fantasy Football Frenzy promotion (the “Promotion”) you may enter via the following method:
ON-SITE ENTRY: Beginning on Monday July 30th, 2012, stop by any of the forty-four (44) participating Chicagoland and Northwest Indiana Buffalo Wild Wings locations attached hereto as Addendum A and incorporated herein by this reference (“Buffalo Wild Wings Locations”), during normal business hours. There is no cover charge at the Buffalo Wild Wings Locations. Upon arrival, you will need to schedule a date to hold your draft party. Entrants must obtain an official entry form (available while supplies last), legibly complete the official entry form in its entirety, which may include without limitation, entrant’s full name, complete address (including city, state and zip code), daytime phone number (including area code), e-mail address (if any), and date of birth; and hand the completed official entry form to the store manager on site. Draft parties must be held by Wednesday, September 5th, 2012 at the close of business. The entry form must contain the names and addresses of a twelve (12)-person team. The team will be assembled at the time of the draft party. At the time of entry, each team must name a team captain and list all team member names. All team members must meet all eligibility requirements listed in these Promotion rules. You may have up to twelve (12) people on your team, including the team captain. All entries must be received by the Buffalo Wild Wings close of business on Wednesday, September 5th, 2012. Late entries will not be accepted.
2. ELIGIBILITY RESTRICTIONS
a. The Promotion is open to all persons who are 21 years of age or older and who reside in the Station’s Arbitron Metro Area, which includes counties in Illinois, Indiana, Michigan, and Wisconsin. Employees of the Station, its licensee, the licensee’s parent, subsidiary and affiliated entities, Station’s advertising and promotional agencies, its participating sponsors, other radio stations in the Chicago metropolitan area, the members of their immediate families (spouse, parent, sibling or child) and those living in the same household of each (whether related or not), are ineligible to enter or win. This Promotion is subject to all applicable federal, state and local laws and regulations and is void where prohibited.
b. One (1) entry per team.
c. Entrants are permitted to be listed as members of more than one (1) fantasy football team.
d. The winner must provide valid government-issued photo identification and provide their complete address, date of birth and phone number to claim a prize.
e. Entrants are required to provide truthful information and the Station will reject and delete any entry that it discovers to be false or fraudulent. The Station will disqualify any entry from individuals who do not meet the eligibility requirements.
3. PRIZES
a. One (1) grand prize will be awarded and up to eleven (11) secondary prizes will be awarded.
The pre-determined captain of the winning team will receive one (1) grand prize. The grand prize consists of the following:
(i) One (1) trip for two (2) people to Las Vegas, Nevada. Including round trip coach airfare for two (2) people (winner and one (1) guest, must be 18 years of age or older) from Chicago, Illinois to Las Vegas, Nevada.
(ii) One (1) two (2) nights/three (3) days stay at to be determined hotel located in Las Vegas, NV. Room is a standard, double occupancy room.
The Approximate Retail Value for the grand prize (“ARV”) is $2,000
b. The remaining (up to) eleven (11) pre-determined team members of the Winning Team will each receive one (1) the following secondary prizes: One (1) $25 gift certificate valid at Buffalo Wild Wings. Certain Restrictions apply. The ARV of each secondary prize is $25. The gift certificate will be subject to the terms and conditions as set forth by the issuer of the gift certificate. Redeemable for products at any participating Buffalo Wild Wings location. To find a location near you, visit www.buffalowildwings.com. This gift card may be used toward the purchase of food, beverage or alcohol where permitted by law, in the USA only. A balance will remain on the gift card until the full amount has been used except where otherwise required by law. This gift card has no service fees or expiration date. The gift card cannot be replaced if lost or stolen.
c. Certain travel restrictions may apply. Travel arrangements must be made through the Station. Grand Prize is subject to availability. If the Grand Prize winner cannot travel on dates specified by the Station, the winner may select a different team member to go in his/her place. All other costs not specifically stated herein, including but not limited to those of, taxes, meals, tips and ground transportation to/from the airport and/or to/from the hotel, are the responsibility of the winner. The winner and guest must travel on the same itinerary and are responsible for obtaining all required travel documents. If applicable, the winner may be required to present a valid credit card upon hotel check in to cover any incidental expenses incurred during the winner’s stay. The winner must sign a Publicity, Liability Waiver and Travel Release form prior to traveling. If a selected event is unable to take place as scheduled, for reasons such as cancellation, preemption, postponement or unavailability, or for any reason beyond the control of the Station, the Station’s sole responsibility to the winner will be to award the remaining available portion of the prize and no substitution or compensation will be provided for the unawarded portion of the prize.
d. All prizes or prize certificates may be picked up at the office of the Station at the address below. The winner will forfeit any prize or prize certificate not claimed within ten (10) days of winning. In the event that a prize or prize certificate is mailed to the winner, it will be with the prior written consent of the winner and therefore, winner assumes the risk of its loss. The Station is not responsible for the safe arrival of a prize or prize certificate. The pick-up is prior to the trip dates.
e. There is no substitution, transfer or cash equivalent for prizes, except that the Station may, at its sole discretion, substitute prizes or cash of comparable value. The prizes are expressly limited to the item(s) listed above and unless otherwise expressly specified, do not include taxes, gratuities, parking or any other expenses. Other restrictions may apply.
4. WINNER SELECTION AND NOTIFICATION
a. Decisions of the Station management with respect to the Promotion are final.
b. On Friday, September 28th, 2012 at approximately 12:00pm CT, the Station will select one (1) team per Buffalo Wild Wings Location where entries are accepted through a random drawing from all entries received to be in the drawing for a chance to win the grand prize and secondary prizes. Teams selected through this drawing will be deemed “Qualifying Teams”, upon confirmation of eligibility. There will be a total of forty-four (44) Qualifying Teams (one (1) per location) selected.
c. At 2pm CT on September 28th, 2012, the Station will then select one (1) Prize Winning Team, through a random drawing from all Qualifying Teams. The Station will notify the Winning Team by phone following the Prize Winning Team drawing. The grand prize and secondary prizes will be awarded to the Winning Team members upon confirmation of eligibility. The grand prize will be awarded to the captain of the Winning Team and the secondary prizes will be awarded to the other members of the Winning Team.
d. Odds of a team being selected as a Qualifying Team depend upon the number of eligible entries received at each respective Buffalo Wild Wings Location. Odds of a Qualifying Team being selected as the Winning Team are 1 in 44.
e. Entrants need not listen to the Station nor be present to win.
f. Each member of the prize winning team must execute and return any required affidavit of eligibility and/or liability/publicity release within ten (10) days of winning or prize will be forfeited and an alternate winner may be selected at random. If a potential winner cannot be contacted, fails to sign and return the required affidavit of eligibility and/or liability/publicity release within the required time period, or if a prize or prize notification is returned as undeliverable, the potential winner forfeits the prize.
5. CONDITIONS
a. Payments of all federal, state and local taxes are solely the responsibility of the winner. The winner will be required to complete and submit an IRS Form W-9 with the winner’s full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by the Station (or any other CBS Radio station in Station’s market) in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize. Such winnings of $600 or more will be reported to the IRS.
b. By participating in the Promotion, the winner agrees to have the winner’s name, voice and likeness used in any advertising or broadcasting material relating to the Promotion without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c. Prior to awarding any prize or prize certificate, the Station, in its sole discretion, may require Promotion winner (and any travel companion(s) or guest(s)) to sign a liability release, agreeing to release and hold harmless the Station, its licensee, the licensee’s parent, subsidiary and affiliated entities, their respective officers, shareholders, directors, employees, agents and representatives and all of their successors and assigns from and against any and all claims or liability arising directly or indirectly from the prize and participation in the Promotion.
d. If for any reason this Promotion cannot be executed as planned, including, but not limited to, as a result of infection by computer virus, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Station that corrupt or affect the security, administration, fairness, integrity or proper conduct of the Promotion, or if the Promotion is compromised or becomes technically corrupted in any way, electronically or otherwise, the Station reserves the right to cancel, terminate or modify the Promotion. If the Promotion is terminated before the original end date, the Station reserves the right to select the winners by random drawing from among all eligible non-suspect entries received as of the termination time/date or otherwise modify the procedure for selecting winners.
e. The Station, in its sole discretion, reserves the right to disqualify any person tampering with the entry process or the operation of the Station’s website. Failure to comply with the rules of the Promotion may result in a contestant’s disqualification and/or forfeiture of any prize or prizes. If the Station makes a good faith determination that an entrant has cheated or committed fraudulent activity in connection with a Promotion, the Station reserves the right to disqualify that entrant from entering and/or winning future Promotions and to prosecute and seek damages to the fullest extent permitted by law.
f. The Station reserves the right to make changes in the rules of the Promotion, including, without limitation, the substitution of a prize of equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of the Station, any competition or prize-related event or travel is delayed, rescheduled, postponed or cancelled, the Station reserves the right, but not the obligation, to cancel or modify the Promotion and shall not be required to award a substitute prize.
g. The Station is not responsible for typographical or other errors in the printing, the offering or the administration of the Promotion or in the announcement of a prize.
h. For the names of the prize winners send a self-addressed stamped envelope for receipt by December 28th, 2012 to the above address marked Buffalo Wild Wings Fantasy Football Frenzy– Winner List.” The Official Rules and the Winner List (when completed) shall also be available during regular business hours at the main offices of the Station and shall be posted online at www.670thescore.com
