*Job Summary/Essential Duties:*

* *Provide initial care coordination*
o Initially review the referred patient’s diagnosis, medical history, and physician disposition and effectively communicate this information to the patient and physician
o Provide extensive and effective education to our patients
o Assist physicians in coordination of care with other health care professionals.
o Refer patient to other community resources, as necessary
o Communicate physician disposition and/or patient response to recommendations to the primary care and referring physician
* *Provide ongoing care coordination*
o Follow-up with patient and interdisciplinary team in a timely manner to discuss next step treatment, as well as obtain ancillary documentation of patient tests and visits outside ACH.
o Communicate progress to the reviewing physician for next steps determination.
o Immediately communicate patient non-compliance with treatment recommendations to the reviewing physician
o Communicate treatment information to the primary care and referring physician if pre-treatment results in symptom resolution and patient does not require further follow up
o Accurately provide on-going documentation of care coordination activities on each patient, including time involved in these activities
o Performs in-patient rounding independently and in attendance with physician if requested.
* *Coordinates and executes nursing navigator responsibilities*
o Serves as a clinical expert to help patients and families “navigate” the healthcare system and insurance process.
o The navigator assists in development and performance of specialty services, educational activities, program evaluation and outcome research.
o Works with physicians, clinical staff and community and advocacy groups to enhance coordination of patient care across the continuum.
o Oversees patient care delivery to maximize patient/ family/ physician satisfaction. Acts as an advocate for patients and family members. Acts as a resource for staff.
o Maintains, participates, and demonstrates work practices consistent with established departmental policies, procedures, and objectives concerning quality improvement, safety, security, environmental conditions, and infection control.
o Assumes responsibility for personal and professional growth through participation in departmental meetings, in service programs, and continuing education programs. Fulfills yearly educational requirements as stated by policy.
o Maintains professional attitude with physicians, patients, families, and other hospital personnel while assuring confidentiality of patient and other hospital information.
o Complies with internal and external regulations, policies, and procedures. Participates in achieving surgery division goals.
o Acknowledges the value of multicultural traditions and demonstrates this when caring for patients and family members.
o Appropriately delegates nursing procedures which do not require the skill/knowledge or judgment of a licensed professional and which may be safely delegated in accordance with nursing policies and procedures. Supervises the provision of care by non-licensed personnel as necessary.
o Educates the patient, family and staff by providing facts about the pre and post surgical experience, ongoing treatment plans, and available resources.
o Assists with community outreach as necessary.
o Participates in Quality improvement by tracking interventions and outcomes as needed.
o Performs other duties as assigned
o Manages case load of active and follow up patients.
o Main point of contact for patients and families and is the liaison between patients and the surgeon.
o Performs/directs patient care services including, but not limited to, admission interview, assessment, accurate documentation and timely patient flow.
o Triage patient care to ensure priority is provided to patients in an emergency situation. o Ensures patient is appropriately prepared for care encounter. Prepares/obtains patient medical records and other information/documentation pertinent to patient encounter.
o Assists physician/healthcare provider with exams, treatments, procedures and testing as directed, Ensuring appropriate consent forms are obtained as required.
o Schedules and provides patient instruction(s) for tests, treatments and hospital admissions, following correct insurance protocol. (Ie. Precertification, referrals, chart documentation, etc.)
o Provides patient and family education pertaining to healthcare management, disease process, treatment, test(s), medication and follow-up. Maintain/update patient education materials and consent forms.
o Triage patient telephone calls utilizing knowledge of professional nursing theory and practice.
o Receives, prioritizes and responds to incoming calls as appropriate.
o Administers prescribed medications, as ordered by the physician/healthcare provider, up to and including I.V., in accordance with nursing standards.
o Performs appropriate orders for lab procedures and communicates results to the patient as ordered by the physician/healthcare provider
* *Participates in departmental, interdepartmental and organizational activities, and accepts educational opportunities*
o Assumes responsibility for personal and professional growth through participation in clinic meetings, in-service programs and continuing education programs. o Responsible for additional projects and/or special assignments, serves on committee/task force as assigned.
o Participates in all safety, security and infection control programs that are mandatory as well as those required and provided by the department.
o Demonstrates work practices consistent with Alegent Creighton Health and department-specific safety, security and infection control policies.

o Performs all related clinic functions
+ Prepares rooms for daily patient care
+ Responsible for sterilization; restock instruments
+ Cleans and resets rooms between patients
+ Orders drugs and supplies
+ Helps orient new employees to clinic through explanation of procedures
o Maintains compliance with established departmental and organizational policies, procedures and objectives concerning quality improvement, safety, environmental conditions and infection control.
o Authorized to make entries in the medical record, consistent with and limited to the scope of the job duties and any applicable licensure.
o Must have ability to work effectively and collaboratively in a diverse and multi-cultural environment.

Hours: Flexibility will be key with hours as this may require some evening and weekend phone calls and care coordination.

**Qualifications:**

*Experience*

Required: One or more years of clinical nursing experience in an ambulatory health care setting.

Preferred:Case Management experience

Electronic Medical Record experience

Awareness of surgical process

*Education/Licensure*

Required:

Licensed in Nebraska or Iowa as a Registered Nurse, Nebraska and Iowa are compact states must be licensed in the state of your residence.

BLS or BLS instructor certification is required upon hire. Acceptable credentialing bodies and certifications include the following: American Red Cross: CPR/AED for the Professional Rescuer, American Safety and Health Institute: CPR, PRO, must say “Professional Level-with AED”, and American Heart Association: Basic Life Support for Healthcare Providers.

Preferred:Bachelor of Science degree in nursing or will obtain within 2 years.
