**Description:**
Job Summary:
Provides in-home visitation services to first-time parents and/or families beginning at pregnancy to the age 3, based on program curriculum and fidelity guidelines. Works closely with SJC Enhanced Referral Services Program staff to ensure successful connection of children/parents/families of home visiting program with needed resources and services.

Essential Duties:

1. Conduct home visits weekly and schedules visit times to meet the needs of families and the program (which may include some early morning, evening and Saturday visits as approved by supervisor). Hours may vary and Home Visitor must be flexible and accountable for managing time daily in an efficient and effective manner. Meets SJC program goal for number of home visits weekly as assigned by Program Manager and Team Leader. Provides regular shadowing / training opportunities for newly hired staff in the program.
2. Achieve competency in all training protocols.
3. Follow program curriculum and guidelines established by the program and SJC, including SJC values and religious and ethical guidelines.
4. Maintain confidentiality of information at all times; obtain appropriate release of information as needed.
5. Connect parent/family with SJC Enhanced Referral Program as applicable.
6. Appropriately represent SJC in all interactions. Completes outreach assignments as determined by the Team Leaders and Program Managers
7. Maintain family case records in the database; insures that all notes, summaries, and data base entries are accurate and up-to-date and entered within 5 working days of the home visit, and provide timely and accurate reports.
8. Effectively meet needs of parent/family and child, respecting strength and preferences of parent/family.
9. Acts and serves as an Advocate for young children and families

Skills, Knowledge or Abilitiescritical to this role:
· Achieve competency in all protocols, curriculum, record-keeping, database use.
· Perform duties within approved scope and in keeping with SJCH values and religious and ethical directives.
· Demonstrate excellent interpersonal communications skills.
· Ability to remain calm in difficult situations.

**Qualifications:**
Education / Accreditation / Licensure:
Required: High School Diploma. Current NM driver’s license and current liability insurance coverage.

Preferred: Associate or Bachelor’s Degree in early childhood education / child development field, elementary education, or other related field.

Required: Bilingual – able to deliver the curriculum in Spanish. Experience working with pregnant women and/or families and/or young children / population served by home visiting program. Ability to maintain accurate records and correspondence.

Preferred: Prior home visiting experience. Prior experience in child care setting.

Other: Required - Must have regular access to reliable transportation; clean driving record, current NM driver’s license and current liability insurance.
