CHI St. Luke’s Health is a part of Catholic Health Initiatives (CHI), one of the nation’s largest health systems. Headquartered in Englewood, Colorado, CHI operates in 19 states and comprises 105 hospitals, including four academic medical centers and teaching hospitals; 30 critical-access facilities; community health services organizations; accredited nursing colleges; home health agencies; and other services that span the inpatient and outpatient continuum of care.

POSITION SUMMARY
Develop and implement initiatives to improve community health in the Houston area market based on current best practices and emerging community and population health models. Forge and nurture community partnerships in the Houston area market that demonstrate measurable metrics, quantify collective impact and support organizational strategy. Facilitate Houston area market support for global initiatives to build healthy communities for the poorest and most vulnerable people in developing countries in partnership with others

POSITION RESPONSIBILITIES
1. Oversee and direct (1. the completion of the CHNAs for CHI St. Luke's facilities and (2. the development and implementation of resulting strategies and reporting requirements in collaboration with the VP MI and other market leaders to ensure compliance with all regulatory requirements.
2. Represent CHI St. Luke's Health in Houston area market community coalitions, collaborative and organizations that promote community health, or identify and support organizational leaders in doing so.
3. Work with the senior market leaders and VP MI to monitor population health and clinical redesign efforts in support of effective medical management across the care continuum which effectively interfaces with local community health initiatives.
4. Strategically link Houston area market leaders, staff and physicians with healthy community initiatives, building CHI St. Luke's image and value while providing meaningful volunteer experiences for our staff.
5. Manage internal and external resources necessary to effectively carry out healthy community initiatives.
6. Conduct evaluations and complete reports to measure the effectiveness of healthy community initiatives based on health outcomes.
7. Assure organizational compliance with federal and state community benefit documentation and reporting requirements in collaboration with our Finance and Tax departments.
8. In furtherance of CHI's strategic initiative, advance CHI St. Luke's Health commitment to non-violence as an essential element of healthier communities through public policy initiatives and community based programs in the Houston area market.
9. Serve as an organizational lead on advocacy issues.
10. Serve as an organizational lead on community relations initiatives, representing CHI St. Luke's Health in an array of capacities and aligning activities to support our strategies.
11. Model servant leadership and participate in committee work and task forces within the health system.

**Qualifications:**
Minimum Job Qualifications:
*Minimun of seven (7) years experience in the discipline and five (5) years leadership experience

Minimum Education:
*Master's Degree
