*Job Summary:*
Provides pharmaceutical care services to all patients and customers of GSH. Responsible for the filling and dispensing of medication orders including the proper handling, usage, and storage of drugs in the pharmacy.
Maintains standards set by the department and translates policies and procedures into the daily operation of the pharmacy. Responsible for acting as an information resource to patients, physicians, nurses and other healthcare professionals regarding medication therapy and drug-related information and literature. Plans and organizes work schedule of supportive personnel as required. Under the direction of the Clinical Coordinator and the Director, optimizes patient medications therapy by providing comprehensive clinical pharmacy services. Works collaboratively with the health care team to accomplish goals from a centralized and decentralized area of the hospital
*Essential Duties:*

* Performs all responsibilities/duties required by the pharmacy as defined by the scope of service to ensure that the unique nature of the client is addressed
* Responsible for the preparation and distribution of medication orders filled for the inpatient, outpatient and ancillary areas. Turn around times are met consistently.
* Responsible for ensuring the correct labeling of medication containers.
* Responsible for interpretation of physician orders for accuracy and appropriateness, informing them of any errors, omissions, or discrepancies and, based on an evaluation of the patient’s condition, suggesting appropriate or alternate drug therapy to the physician.
* Responsible for the development, preparation and compounding of special dosage forms of drugs not commercial available.
* Responsible for the daily coordination of the manufacturing and distribution of sterile products, including intravenous admixtures, TPNs, chemotherapy and miscellaneous products.
* Responsible for inventory control/purchasing activity as required to maintain departmental inventory at appropriate levels. Responsible to maintain, inventory and handle all drugs, including investigational drugs according to federal, state, hospital and departmental policies.
* Completes all chemotherapy procedures including the chemo QA checklist and labels all chemo products appropriately.
* Responsible for maintaining accurate and proper patient medication profiles, monitoring to insure the proper choice of drug, dosage form, strength, length of therapy and interaction potential of the prescribed drug.
* Maintains and updates the hospital formulary system according to hospital policy and suggests alternative medications to the prescriber when non-formulary drugs are ordered.
* Responsible for special projects pertaining to specific areas of interest and/or expertise; for conducting in-depth and highly specified research reviews of drug-related literature and ability to make presentations on rational drug therapy.
* Responsible for counseling patients per policy and upon request.
* Provides clinical pharmacy practice services including primary healthcare, consultative services and drug information services to patients and other healthcare providers directly and through committee work.
o Ensures medications are correctly dosed based on patient’s renal or liver function.
o Ensures cost effective therapies by recommendations for IV to oral switches.
o Ensures optimal antibiotic therapy via review of Culture and Sensitivity reports.
o Streamlines antibiotics for cost effective therapy.
o Sustains the hospital drug formulary, minimizing nonformulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.
o Suggests appropriate, cost effective therapeutic alternatives to medical staff.
o Provides clinical consultation and clarification to practitioners.
o Detects and reports suspected adverse drug reactions accurately and in a timely manner.
o Is competent in pharmacokinetics. Accurately completes all Therapeutic Drug monitoring and documentation
Hours: Mostly days (8 or 9.5 hour shifts), one evening a week. Saturday and Sunday shift every 4th weekend.
**Qualifications:**
* Education: *
• Must be a graduate of an accredited college of Pharmacy with a Bachelor’s degree in Pharmacy.
• PharmD preferred.
*Licensure:*
• Must be licensed to practice pharmacy in the State of Nebraska.
• Basic Life Support Certification is required prior to clinical work on the nursing units.
*Experience:*
• No prior experience is required.

• Must have the ability to communicate effectively with all people and project a positive attitude toward work and people.
• Attention to detail is essential.
• Required to understand proper handling and disposal of chemotherapy drugs, departmental safety policies and universal precautions.
