*Job Summary:*
Responsible for the coordination, development and maintenance of activity program, utilizing staff expertise and family and community resources. Plans and carries out an activity program which encourages residents to function at the highest level of independence possible. Assists residents aged pediatric, adolescent, adult, and geriatric to participate in activities of a recreational, diversional, religious, intellectual and work-type nature to promote mental and physical stimulation and socialization opportunities.
*Essential Duties:*

* Interviews and assesses all residents within seven days of admission.
* Develops and updates individual activity plan with measurable objectives consistent with resident's care plan and physician order.
* Maintains documentation of resident's progress.
* Organizes the Activity Program to meet state and federal expectations.
* Provides varied activities which is congruent to needs/interests of residents and takes resident to the activity by pushing wheelchair.
* Communicates and interprets the activity program to prospective residents, families, community groups and staff.
* Organizes monthly activity calendar and have ready to copy before the first of the month.
* Evaluates the overall effectiveness of the Resident Activity Program and as necessary, modifies the program to meet resident needs/interests.
* Attends and contributes to weekly Interdisciplinary Care Plan meetings.
* Recruits, screens, orients and supervises volunteers. Coordinate the Activity Program with available volunteers.
* Completes nursing documentation on a daily basis via computer.
* Maintains knowledge of current trends and developments in the field by reading appropriate books, journals, and other literature and attending related seminars and conferences.

Hours: 0700-1530; 5 days a week activities

**Qualifications:**
* Minimum Knowledge, Skills and Abilities Required: *
1. High school graduate with minimum of one year of experience in a social or recreation program in a long term care setting within the last five years. Ability to update/document all patient activities and plan of care on computer.
2. Completion of 36 hour Activities' Director Course established by Nebraska Health Care Association is preferable. A sincere interest and desire to work with long-term care residents.
3. Interpersonal skills necessary in order to establish effective working relationships with the nursing staff, volunteers and residents.
4. Analytical skills necessary to gather and interpret budget reports, volunteer participation and program effectiveness.
5. Must have and maintain at all times, a valid Nebraska drivers’ license and a good driving record. Must be able to drive the Skilled Care van for patient rides and doctor appointments.
