Exciting Opportunity:
As a member of a multi-disciplinary Mental Health and Addictions Team providing a comprehensive array of services within a psycho social rehabilitation framework to adults with serious and persistent mental illness and/or within a harm reduction framework for those with substance use problems/disorders and who experience severe challenges in daily living as a result. This position will provide these services offering a variety of rehabilitative and treatment modalities inclusive of outreach, community and facility care.
The Life Skills Support Worker will:
• Provides direct care such as supportive counseling, risk assessment and crisis intervention for individuals or groups who experience severe challenges in daily living as a result of their mental illness and/or substance use problems/disorders.
• Participates in the development, coordination, implementation and evaluation of a treatment plan via an outreach methodology for assigned clients. Participates in team clinical reviews and consultation, psychiatric reviews and the monitoring of clients.
• Provides support via an outreach methodology that is inclusive of: access to food, housing, medical care, social/recreation activities, supported work and employment opportunities. Provides life skills training such as money management, self-care and nutrition information. Participates in psychosocial rehabilitation and/or addictions recovery activities in the community and in addiction treatment and recovery facilities.
• Supports, develops and maintains positive working relationships with all agencies and service sectors that provide services for those with mental illness and/or substance use problems/disorders.
• Provides support and advocacy services, such as supporting clients in the legal system and landlord tenant disputes, refers clients to services and follow-up to ensure they are receiving the care that is required. Works in conjunction with the Office of the Mental Health and Addictions Advocate for the Kootenay Boundary Health Service Area.
