CPT Online Exam Form Procedure
Step 1:
Create Your Account: for every upcoming exam by clicking at the link “If you are a new user, please click here” (The users who had created login account for any previous exams will have to re-create the account) and authenticating your Student Registration Number (eg: NRO0123456) with your date of birth, your VALID email-id and mobile number. (In Case your Name is not displayed on entering the registration number and Date of Birth, you can still create the login as a PROVISIONAL CANDIDATE by providing correct particulars. You must submit the copy of registration letter and other relevant documents with your application form in case your login is created as Provisional Candidate.)
Double check your email-id: provided by you in the above step, since the login-id and password created by you for filling up the application form would be sent to this email address and mobile only.
Step 2:
Login and Pay Fee Online: Login here with the username and password you receive in your email ID/mobile as provided in Step 1 above. Fill up the Online Application form carefully, confirm the particulars displayed in the Confirmation Page by tick marking in all the boxes and then clicking Confirm button, and Pay the Fees Online using Master / Visa / Maestro Credit or Debit card.Click Here For Fee Details (For general instructions and guidance on how to fill the form, please refer to the “Guidance Notes” given above at the home page) In case you are using a Debit Card, ensure that it is either Master / Visa / Maestro Debit Card.
Step 3:
Login Again to Take print out of PDF form on A4 size paper with Portrait orientation. Do Not print on both sides of the paper. If PDF is not generated Click Here.Check and ensure that Control Number, Bar code and Payment Particulars are appearing correctly in PDF before taking the print out. In case of any issues please report to onlineexamforms@icai.in
Step 4:
Affix Two Photographs & Signature: Paste your Photographs, Sign the form using Black Ink Pen and get the form attested by Principal/Member of the Institute/gazetted officer, as mentioned in the form printed in Step 3 above. One photograph should be attested and the other one not attested
Step 5:
Send us the printout: The Last Date for Receipt of Printout of Exam Application form (PDF) filled Online after affixing photographs and specimen signature and signing the declaration duly countersigned by your parent / gurdian and attested by a Chartered Accountant or Gazetted officer or Principal of school or college is 02-05-2015
Additional Secretary (Exams)Submission by Post : You may send printout of the form by SpeedPost/Registered Post (not courier) without folding and Superscribing the envelope as “Exam Application Form CPT, June – 2015” so as to reach us on or before 02-05-2015 . The address for posting is as under:

Important Queries Related to CPT Exam Form June 2015
What is the last date for submission of forms?
Last date for submission of on-line/physical (OMR) forms is — 28-04-2015( Upto 17.30 Hrs IST)
Last date for receipt of the print-out (PDF) of the online exam forms is — 02-05-2015 ( Upto 17.30 Hrs IST)
I do not have regular access to internet or computer? Is it compulsory to fill the on-line form? Or can I use the physical form?
Though it is not compulsory to fill the on-line form, it is strongly recommended that you may preferably fill the on-line form, if possible. Alternatively, you can use the physical form
Why should I use the on-line form when I can fill the physical form?
You will be paying less, if you apply on-line as against physical forms. Details of the amount of fee payable are available in answer to Question No. 2 above.
Ease and convenience. You can apply from anywhere, any time at your convenience,( within the stipulated dates and time). In the case of on-line forms, payment of examination fees is also to be made on-line, through credit/debit cards. No need to go anywhere to buy the form. However, you have to despatch the printout of online application form, duly attested, by Speed Post/Registered Post to the Institute so as to reach us latest by 2nd May, 2015
I have not received any email or SMS indicating my log in ID or password though I have already registered myself?
Please check the bulk/spam mail folder for a recent mail with the subject “login ID password for ICAI examination”. I forgot my password. What should I do?
Click on the “Forgot/did not get password” on the login screen. After selecting the exam applied and entering your registration number and date of birth, the email will be resent to you. I submitted my Registration Application Form for joining the Common Proficiency course in time but I have not yet received my Student Registration Number from ICAI. How should I apply for the June, 2015 CPT?
You can submit your provisional on-line application.
Also, contact the concerned Regional Office of the Institute and get your registration number. Thereafter, you may submit a copy of the your registration letter to the Exam Department at the address given below (enclosing therewith a copy of your online application form), by separate post, along with a signed letter quoting your registration number and bar code/control number of your exam application form requesting for taking your correct Registration Number on record before hosting of Admit Cards.

In case you fulfill the eligibility conditions as mentioned in “Guidance Notes” and you have applied for Common Proficiency Course registration on or before 1st April, 2015 but have not yet received your registration number OR for your registration number the online exam portal is indicating “Already Registered” and you wish to apply online, you may apply as per the example given below:
Example Name: ASHISH KUMAR SINGH
Example Date of Birth: 01-10-1997
Then you may apply against registration number ASHI011097 (first four characters of name + Date + Month+Year of Birth)
I do not have credit/debit cards. What should I do?
Someone else can pay for your test, as long as you have their permission to do so.
Take care to give your details when filling in the on-line form. When paying for your exam, we’ll ask you for the cardholder’s details
Can I use internet banking? How do I pay the exam fees on-line?
No. Payment of exam fees through internet banking is not permitted. You can make payment of exam fees by using master/visa/maestro credit/debit cards only.
The word PROVISIONAL is appearing on the print-out of the on-line form. Why is it so?
The word provisional is appearing because while creating your login-id, your registration number and/or date of registration and/or your date-of-birth did not match with particulars in our database or you did not enter your correct student registration number and/or date of birth.
You may take a print out of the provisional exam application form and send us in the normal course, as per the procedure prescribed.
Also send seperately, a signed letter quoting the registration number and bar code number/control number, with a copy of your Common Proficiency course registration letter, to us to verify your registration number, by separate post. Please do not send the request letter along with the print out of the on-line form. Send it by separate post.
If you have not received your registration letter from the concerned regional office of ICAI so far, you may fill in and submit your exam application form as Provisional ( Refer Q14 above) and later on submit a copy of the same as soon as you receive it. Do get in touch with your regional office to confirm your registration number and date of birth.
Do I need to send any documents along with the print out of my on-line form?
No. Please do not send any enclosures along with the printout of on-line form.
