Eligibility
Delivered Dish Halloween Costume Contest (the Contest) is open to legal residents of Oregon, California, Colorado, New Mexico, Nevada, and Texas who are at least eighteen (18) years old at the time of entry. Employees of Delivered Dish and other companies associated with the promotion of the Contest, and their respective parents, subsidiaries, affiliates and advertising and promotion agencies as well as the immediate family (spouse, parents, siblings, and children) and household members of each such employee are eligible to participate. Grand Prize winner must be within a zip code of Delivered Dish’s service area. The Contest is subject to federal, state, and local laws and regulations.

The information you provide will only be used for marketing purposes by Delivered Dish only. We do not sell your information to third party sites.

Agreement to Official Rules
Participation in the Contest constitutes entrants full and unconditional agreement to and acceptance of these Official Rules and the decisions of Delivered Dish, which are final and binding. Winning a prize is contingent upon being compliant with these Official Rules and fulfilling all other requirements set forth herein.
Contest Period
The Contest begins on October 1, 2015 at 10:00am PST and ends on November 1, 2015 at 12:01am PST (the Photo Contest Period). Your submission must be placed before 9:00pm October 31, 2015 and all eligible votes must be in before 12:01 am PST on November 1, 2015.
Entries that are submitted before or after the Photo Contest Period will be disqualified. Submissions will be accepted for the duration of the Photo Contest using any of the following methods:
Facebook entry, email entry, blog entry, hashtag entry (Facebook, Twitter & Instagram).
How to Enter
(a) Facebook: This method of entry will be available by visiting our Facebook page (Facebook.com/Delivereddish) and clicking on our Halloween Costume Contest tab. To enter, complete the entry form with the required information, including your name, email address, and photo caption (optional) and submit along with your photograph. Each Submission consists of an entry form and a single image. You may enter the Contest (1) time daily. Each entry must comply with the article entitled “Photo Requirements.”
(b) Email Entry: This method of entry will be available by emailing your original photograph, the email address you would like to use if you would be declared a winner and any caption you would like to appear near the photograph (optional) to Info@ddish.com with the subject “Halloween Costume Contest Submission.” Each entry must comply with the article entitled “Photo Requirements.”
(c) Blog Entry: This method of entry will be available by visiting blog.ddish.com and clicking on the “Halloween Costume Contest” blog post. To enter, complete the entry form with the required information, including your name, email address, and photo caption (optional) and submit along with your photograph. Each Submission consists of an entry form and a single image. You may enter the Contest (1) time daily. Each entry must comply with the article entitled “Photo Requirements.”
Limit one (1) entry per person, per email address, per day, regardless of method of entry. Entries received from any person, e-mail address or household in excess of the stated limitation will be void. All entries become the property of Delivered Dish and will not be acknowledged or returned.
PHOTO REQUIREMENTS:
Photographs must be in digital format. Only online entries will be eligible. No print or film submissions will be accepted for entry into this Contest. All digital files must be in JPEG or PNG format. The photograph, in its entirety, must be a single work of original material taken by the Contest entrant. By entering the Contest, entrant represents, acknowledges, and warrants that the submitted photograph is an original work created solely by the entrant, that the photograph does not infringe on the copyrights, trademarks, moral rights, rights of privacy/ publicity or intellectual property rights of any person or entity, and that no other party has any right, title, claim, or interest in the photograph.
The photograph must not, in the sole discretion of the Contest Providers, contain obscene, provocative, defamatory, sexually explicit, or otherwise objectionable or inappropriate content or in violation of Delivered Dish’s Terms of Service. Disguising or misrepresenting the origin of your content is cause for disqualification. By making a Submission, each Contestant consents to the use of his or her Photo(s), Submission(s), name, likeness, etc. by Contest Providers or Sponsor for promotional purposes in connection with the Contest (whether online, in print or through any other media now known or hereafter developed), without additional compensation and unless prohibited by law.
Contest Providers will not verify receipt of Submissions for Contestants. Contest Providers are not responsible for lost, late or misdirected Submissions.
MULTIPLE ENTRIES WILL BE ACCEPTED FOR THE DURATION OF THE CONTEST.
DEADLINES: The Contest starts at 10:00 AM PST on Thursday, October 1, 2015 and ends at 9:00 pm PST on Saturday, October 31, 2015. Fan voting starts at 10:00 AM PST on Thursday, October 1, 2015 and ends on Sunday, November 1, 2015 at 12:01 AM PST. No responsibility is assumed by Delivered Dish, their partners, or other Contest providers for lost, late, misdirected, or illegible entries or for any computer, online, telephone or technical malfunctions that may occur. All entries become the property of Contest Providers and will not be returned to the Participant. All entries must be submitted from a valid e-mail account that may be identified with a natural person by appropriate independent means, including, without limitation, reverse domain name search. Potential winner may be requested to provide Contest Providers with proof that the potential winner is the authorized account holder of the e-mail address associated with the winning entry.
Fan voting:
Each Photo will then be posted online and shall be accessible via the Website, and public voting will begin on Thursday, October 1, 2015 at 10:00 am PST and continue until 12:01 am PST on Sunday, November 1, 2015 (the “Fan Voting Period”). The public fan vote will determine the winner of the Delivered Dish Photo Contest. Limit one (1) vote per person per email address, per day for the entire Fan Voting Period. Any person voting more than one (1) vote daily for the Fan Voting Period will disqualify that person’s votes for the Finalist. Sponsor is not responsible for any technical/quality issues of the Submissions. The Submission receiving the highest number of fan votes will be deemed the grand prize winner (the “Grand Prize Winner.)
Prize Drawing
PRIZE DESCRIPTIONS: One (1) Grand Prize winner will receive:
– A $100 Delivered Dish Gift certificate that can be used at any Delivered Dish (Portland, OR; San Diego, CA; Denver, CO; Albuquerque, NM; El Paso, TX; Las Vegas, NV; Eugene, OR) locations.
– Their winning photo to be featured on Delivered Dish’s social channels, including but not limited to Facebook, Twitter, Google+, Pinterest, Instagram, and our blog located at www.blog.ddish.com.
Winner Selection:
Winner will be announced on or about Monday, November 2, 2015. The winner will be deemed by the highest number of fan votes from all qualified entries received by the deadline. In the event of a tie, the winner will be selected by the next Submission with the highest amount of fan votes. In addition to meeting the age requirements, the potential winner’s address must be within the stated mileage requirements as determined by at least one of the Google Map results. Contestants that do not meet the mileage requirement will be disqualified.
Winner Notification
The potential winner(s) will be notified by email. Each potential winner will be required to complete and submit a Declaration Of Compliance within five (5) days of the date notice or attempted notice is sent, in order to claim his/her prize. If a potential winner cannot be contacted, or fails to submit the Declaration of Compliance within the required time period, or prize is returned as undeliverable, potential winner forfeits prize. If the potential winner is at least 18 but still considered a minor in his/her jurisdiction of residence, Sponsor reserves the right to award the prize in the name of his/her parent or legal guardian, who will be required to sign the Declaration of Compliance on the potential winners behalf and fulfill any other requirements imposed on winner set forth herein. Potential winners must continue to comply with all terms and conditions of these Official Rules, and winning is contingent upon fulfilling all requirements. In the event that a potential winner is disqualified for any reason, Sponsor will awarded the applicable prize to an alternate winner by selecting a winner from the next entry with highest amount of fan votes. Only three (3) alternate drawings will be held, after which the prize will remain un-awarded. Prizes will be fulfilled approximately seven (7) days after the conclusion of the Contest.
General Conditions
In the event that the operation, security, or administration of the Contest is impaired in any way for any reason, including, but not limited to fraud, virus, bug, worm, unauthorized human intervention or other technical problems, or in the event the Contest is unable to run as planned for any other reason, as determined by Sponsor in its sole discretion, Delivered Dish may, in its sole discretion, either (a) suspend the Contest to address the impairment and then resume the Contest in a manner that best confirms to the spirit of these Official Rules or (b) terminate the Contest and, in the event of termination, award the prize at random from among the eligible, non-suspect entries received up to the time of the impairment. Delivered Dish reserves the right in its sole discretion to disqualify any individual it finds to be tampering with the entry process or the operation of the Contest or to be acting in violation of these Official Rules or in an unsportsmanlike or disruptive manner. Any attempt by any person to damage the website or undermine the legitimate operation of the Contest may be in a violation of a criminal and civil law, and, should such an attempt be made, Delivered Dish reserves the right to seek damages (including attorney’s fees) and any other remedies from any such person to the fullest extent permitted by law. Failure by Delivered Dish to enforce any provision of these Official Rules shall not constitute a waiver of that provision.
Release and Limitations of Liability
By participating in the Contest, entrants agree to release and hold harmless Delivered Dish, and each of their respective parents, subsidiaries, affiliates, advertising and promotion agencies, other companies associated with the Contest, and each of their respective officers, directors, employees, shareholders, representatives, and agents (the Released Parties) from and against any claim or cause of action arising out of participation in the Contest or receipt or use of the prize (including any travel or activity related thereto), including, but not limited to: (a) any technical errors associated with the Contest, including lost, interrupted or unavailable Internet Service Provider (ISP), network, server wireless service provider, or other connections, availability or accessibility or miscommunications or failed computer, satellite, telephone, cellular tower or cable transmissions or computer hardware or software malfunctions, failures or difficulties; (b) unauthorized human intervention in the Contest; (c) mechanical, network, electronic, computer, human, printing or typographical errors; (d) application downloads, (e) any other errors or problems in connection with the Contest, including, without limitation, errors that may occur in the administration of the Contest, the announcement of the winner, the cancellation or postponement of the event and/or the flyover, if applicable, the incorrect downloading of the application the processing of entries application download or in any Contest related materials; or (f) injury, death, losses or damages of any kind, to persons or property which may be caused, directly or indirectly, in whole or in part from entrants participation in the Contest or acceptance, receipt or misuse of the prize (including any travel or activity related thereto). Entrant further agrees that in any cause of action, the Released parties are also not responsible for any incorrect or inaccurate information, whether caused by site users, tampering, hacking, or by any equipment or programming associated with or utilized in the Contest. Entrant waives the right to claim any damages whatsoever, including, but not limited to, punitive, consequential, direct or indirect damages.
Disputes
Except where prohibited, each entrant agrees that any and all disputes, claims and causes of action arising out of, or connected with, the Contest or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate court located in California, Nevada, Oregon, Colorado, New Mexico, or Texas. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, entrants rights and obligations, or the rights and obligations of Delivered Dish in connection with the Contest, shall be governed by, and construed in accordance with, the laws of California, Nevada, Oregon, Colorado, New Mexico, or Texas without giving effect to any choice of law or conflict of law rules, which would cause the application of the laws of any jurisdiction other than California, Nevada, Oregon, Colorado, New Mexico, or Texas.

Information collected from entrants is subject to Sponsor’s privacy policy. We do not sell your information to third party companies.

This promotion is in no way sponsored, endorsed or administered by, or associated with, Facebook. You are providing your information to Delivered Dish and not to Facebook. By participating in this promotion you agree to a complete release of Facebook from any claims. Participation in this promotion is subject to the official rules.
