It what might be a BHW (it's definitely a first for the time I've been a mod here), I found two BHW members that shitlisted each other within a 12 hour span. Rather than post them separately, I asked them both to email me the full story, along with the skype logs.

Synopsis:
I have been hired by MediaConsult for a frontend form design for her website. I was given with existing form(core html form without any styles applied) and asked to do it like a form which is already in the theme. So, i have did the same but she asked to add some fields(Phone/Skype field) and asked to remove one field(SSID/Fedral Tax ID). So, i did the same and accepted the design but there was a change in the font that i have used and the existing one with the theme. I was not able to recognize the font that have been used on the theme so she said that she will ask the theme developer and later on she gave 2 font name. I have used that but she told it is not right one so i have changed that and at last everything looks good and sent the code.
On the due course i was urged her to spend time to finish so she offered half of the payment since i told i will get after finishing because of her busy schedule she paid half. You can see all this on the skype log since there is no other communication medium used than skype. Days went since there is no deadline given nor asked to finished on some date. None of the day she asked about form unless i get into chat. Always i said "hi" "hi" when i say "hi" then only she will speak about form and she was normal until December 25. Note before that she said that she is "Running like mad" "Year end so busy" and even one day she told me "Not Urgent".
When i again say "hi" at the end of December she gone crap. She simply accused me that i took 2 months time for the simple form. Actually i was urged but she refused as she is busy. Meanwhile she told me to rearrange order of the form fields and i did all. Then the real nonsense starts. She told me that the form is not working. As a developer i know it won't work unless we change backend code(Which connects with Database) because of removing (SSID/Fedral Tax ID). Note: I have notified at the time of asking me to remove the field (Reference to skype log November 22, 2014 7:07 AM). She din't noticed it and keep on telling form is not working. Even i have notified second time (Reference to Skype December 25,2014 8:51PM) she doesn't understand and keep on telling it is not working and told that my form is a mess. So, i asked to send backend file but she doesn't send. I also asked to give backend access to check what is wrong there. You can see on skype log after december 25 none of the time she showed what is error. She simply stated it is not working.
How could a developer know what is wrong with the form without knowing anything about the error?? Even though there is nothing went wrong beyond my prediction. At backend they have check SSID as mandatory but on the new form there is no such field so it was error. At last yesterday after i yell that form will work only if it have same field, she told me to keep the fields which are in existing old form. So i make changes and send to her. She updated and i have checked, it was worked but she tells that she doesn't see anything on the backend admin panel so it is not working. How could i responsible for not showing at admin panel(I did only the frontend form and that too worked as i submitted a test data) even i get email to verify account when i send a screenshot she went crap and gone wild. She disputed and even blackmailed that she will open a shitlist. She have to pay me $27, the total cost of the work is $60. But accusing me and threatening me that she will shitlist.

at the start I did send him a png with the design layout plus gave him a login to a wp theme where he can find the CSS to copy in order to match the form with the rest of the site
after 2 months of corrections
either he put the wrong form
or messes up the order of the fields... name... then street... then social security... well never really like the given PSD
after 2 months I freak out now! I have a form on a live site that is not working! I cannot go ahead with my affiliates ... well I am in the end of my patience... I had given him many chances and nothing really worked out beside I cannot waste my time explaining things over and over again!
I did work with the coder in the past... payed him more than fair and even tips on it.. but this here is unacceptable

My views after reading all of the above:
1) MediaConsult - Karthipers did warn you multiple times that changing the front end would REQUIRE changes to the backend. Since I have done extensive .cgi form modifications, what he told you from the beginning was spot on. If the .cgi requires certain fields that are being removed, that code has to be changed. If you are adding fields to the form, the .cgi has to be modified to accept and pass them on - both to the emails as well as however they are being saved. If they are being saved into a flat file (text), then the file it's being saved to probably doesn't have to be changed, but if it's being saved to a database, the database structure absolutely need to be modified for it to work.
2) Although this requirement was requested by MediaConsult, it is seen repeatedly that karthipers was driving forward progress. On multiple occasions karthipers had to message MediaConsult for hours or days to get more info (ie font) or test what was produced. Several times karthipers messaged MediaConsult just to be told she was busy (especially so in December) and couldn't work on it. When karthipers backed off for a period of time to give MediaConsult time in December, when he messaged him again, he was then accused of waiting too long. Last time I checked, Skype works both ways - and if MediaConsult was suddenly bothered because of the delay, she could have messaged back. Looking at the logs, that didn't happen.
3) MediaConsult, you hired karthipers to modify the front-end form - and even after he warned you that it needed the back-end to be modified to work with it, you wanted him to proceed. He even went above and beyond by offering you - actually begging you to let him help make it work - by either providing the .cgi or giving him access to the back-end. The fact that you didn't accept his offer is your failure, not his.
4) MediaConsult, when you tell someone that what he provided, you have to be specific. If it's giving an error, you need to specify what error it's giving. If it's not looking right, you have to be specific. You were specific on the order of the boxes but very vague on what else wasn't working on multiple occasions. Again, he offered to help fix it in the back-end but you denied his offers.
5) MediaConsult, you accused karthipers of being unprofessional but after reading the logs, you were FAR more unprofessional with how you handled this situation. Your client had to chase you to get you to work on it, you were often too busy to be bothered and then your temperament and language at the end while he was still trying to help you was VERY unprofessional!
I don't need to see anymore and my decision is this. Not only does MediaConsult need to drop the paypal dispute but she owes karthipers the remaining balance, $30. Karthipers not only did what he was asked but he went beyond that and even offered to go further and make it all work. If MediaConsult had allowed him to assist, I believe she would have had a working form in the format he desired.
