Mine worked itself loose. I removed the entire fan & had a look & it sees that the plastic blade is just pressure fit to the motor shaft. I pushed it back on but am concerned that it will happen again. I have an override switch installed so I was able to shut it off when I heard the sound of it coming loose so I don't think it was damaged. I was thinking about glueing it but have never had much success gluing plastic to metal especially where forces are involved with a small surface area.
