This staff- pharmacist, per diem, will work in the DH Nashua Infusion Pharmacy. Prior hospital pharmacy experience is requied and sterile product compounding and hematology and oncology experience is preferred. Dispenses medications, monitors patients' drug therapies, and provides consultation to medical and nursing staff.
This staff- pharmacist is trained to work as an integrated member of the Norris Cotton Cancer Center team. While this is a per diem position, regular hours are needed to cover a medical leave from February to May 2016. This pharmacist must be available on Mondays.

1. Evaluates, prepares and dispenses medication orders.

2. Oversees pharmacy support personnel activities related to the accurate compounding and dispensing of pharmaceuticals and redistributes resources as needed to support workload.

3. Ensures safe, appropriate, and cost effective medication therapies for patients. Monitors medication therapy by assessment of therapeutic appropriateness and duplication of the regimen, appropriateness of the route and method of administration, and patient compliance with the regimen.

4. Reconciles medication lists upon transitions of care.

5. Extracts and interprets information in patient charts.

6. Detects and reports suspected adverse drug events, medication errors and near misses.

7. Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, Pharmacy and Therapeutics Committee approved drug use criteria; and promoting rational drug therapy selection.

8. Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost effective therapeutic alternatives to medical staff.

9. Manages medication therapy where defined by protocol.

10.Provides drug information to the professional staff.

11.Assists in discharge planning. Counsels patient/families concerning proper drug administration, potential drug side effects and precautions, and discharge medication orders. Helps locate medication supplies not generally available.

12.Documents all clinical activities and interventions.

13. Participates in the quality improvement and medication use review activities of the department.

14. Participates in the development and presentation of orientation, education, and training programs for the pharmacy, medical, nursing and other staff members.

15. Oversees and directs Pharmacy support personnel. Verifies the daily activities of assigned staff. Provides input into support personnel performance reviews.

16. Ensures readiness for regulatory and accreditation surveys.

17. Precepts new hires and pharmacy students

Performs other duties as required or assigned.

Minimum Qualifications:

ü Graduate of an Accreditation Council for Pharmacy Education (ACPE) School of Pharmacy with a Bachelor's of Science in Pharmacy required.

ü Doctor of Pharmacy (Pharm.D.) degree preferred.

ü Completion of a post-graduate residency preferred.

ü Previous health system experience preferred.

ü Must be able to work independently with minimal direct supervision, to set priorities, make critical decisions, respond quickly to emergency requests, handle frequent interruptions, and adapt to changes in workload.

ü Good written and verbal communication skills are essential.

Required Licensure/Certification Skills:

ü Must be currently licensed to practice in the state of NH.
