This is an exciting time to join Aetna's Medicare-Medicaid Alignment Initiative (MMAI) team! It is opportunity to help launch a new program that encompasses members who are dually eligible for Medicare and Medicaid.Proficiency in the Microsoft Office Products: Outlook, Word, and Excel or required, as well as a minimum of 2 years experience with electronic charting. A health care professional, an LCSW or LCPC, with a minimum of two years of prior direct service experience is also required for this role.

The ideal candidate is passionate about the Medicaid and Medicare dual population, and is flexible, results oriented, and familiar with psycho/social-specific community resources.There are two openings for this position.Candidate #1:will travel daily to members' homes, supportive living facilities, and nursing facilities to complete comprehensive functional assessments in either the Cook, Lake, Kane, Dupage, Will, or Kankakee County areas. Reporting to the downtown Chicago office, or Downers Grove office depending on location, 2-3 times per week for meetings and trainings. The use of a working vehicle, a valid driver's license, and car insurance is required.Candidate #2:will work fulltime in Aetna's downtown Chicago office with potential travel in the field in the future to members' homes and local facilities.

POSITION SUMMARY
Utilizes advanced clinical judgment and critical thinking skills to facilitate appropriate physical and behavioral healthcare and social services for members through assessment and member-centered care planning, direct provider coordination/collaboration, and coordination of psychosocial wraparound services to promote effective utilization of available resources, optimal member functioning, and cost-effective outcomes.

Involves daily travel going into members' homes, supportive living facilities, and nursing facilities to complete comprehensive functional assessments.Specifically, experience in working with adults with physical disabilities, mental illness, or older adults with disabilities, with a focus on physical and behavioral health issues is preferred as this will be the member population for this position.EDUCATION
The minimum level of education required for candidates in this position is a Master's degree.
